2 Who We Are Location of Facility: Vancouver, BC
Number and type of Patients/Residents/Clients: 45-bed unit that provides therapy and medical care to assist patients to return back to home following a hospital stay. Range from 19 years to 100+ years of age Clients with various medical conditions such as diabetes, CHF and COPD, as well as those recovering from surgery (primarily orthopedic and cardiac), acquired brain injury, stroke, trauma resulting in multiple or complicated fractures and deconditioning due to prolonged hospitalization

4 AIM Reduce incidence of falls (fall rate ) by 40% from baseline
Reduce injury from falls by 40% from baseline by March 2011

5 Change Ideas List Changes you have tested during Falls VLC PDSA Cycles: Added question, “Were there any falls in the last 24 hours?” to morning interdisciplinary huddles. Implemented trial of Fall Prevention Care plan form Introduced a visible identifier for patients identified at risk for falls.

Lessons Learned/Key Insights Staff commitment to participate with the project improved as positive results were observed over time For successful and sustained implementation, education needs to be continuous and multi-faceted Workload and competing pressures made opportunities to collaborate as a full team difficult. Sustainability of the change requires shared responsibility
