Focus: Comprehensive services & program approach to health and wellness Physical, Mental, & Dental health Medical Services (i.e. nutrition, exercise, smoking cessation, asthma management, immunizations) Psycho-educational groups (anxiety, stress reduction, anger management, bereavement) Therapeutic Groups (i.e. NAA/SA/AA) Family and Individual Counseling Specialty Service Specific to one area of medicine (i.e. OB/GYN, STD, Immunization, Fertility, Obesity, Asthma…) Excludes mental health Services typically are not shared with other health care providers

US Army Veteran- Trained in combat life saving Graduated Northwestern BSN program in 2012 5 years as an RN in Trauma, ER, and ICU Master’s Degree University of South Alabama Certified as a Family Nurse Practitioner & Adult-Gero Acute Care Nurse Practitioner. Specialized training as an Emergency Nurse Practitioner.

Latoya Watson, MSW, LCSW Earned a Masters in Social Work in 2009 from the University of Connecticut Licensed Clinical Social Worker since 2014 for the State of CT Trained in Eye Movement Desensitization Reprocessing (EMDR) Therapy and Dialectical Behavioral Therapy (DBT) Formal training in crisis intervention and work history with Mobile Crisis teams throughout the state

Earned a Bachelors in Psychology Currently enrolled in a MSW program at University of St. Joseph Familiar with community resources Point person for referrals to the GHC

Service Model Affordable: Primary Care and Walk-In services
Integrated Care: Behavioral & Medical Services Contracts with area insurers for fee-for-service billing, similar to a doctor’s office. Same day appointments Addresses issues of cost and access, as barriers to seeking health care

Primary care services and Preventative health care services physical exams, immunizations, laboratory tests, nutrition and exercise counseling, weight management, cholesterol management, hypertension, and specialty referrals Acute care of common injuries and illnesses Cold, flu, allergies, conjunctivitis, ear infections, sore throats, sinus infections, skin problems, lacerations, Incision and Drainages, gastrointestinal issues, diagnosis and treatment of sexually transmitted illnesses, and asthma All lab work done at GHC is processed with Quest diagnostics

FAQs What is a copay? A flat fee you pay for certain covered services such as doctor or therapy appointments. What is a deductible? The portion of submitted charges applied towards your deductible. Your deductible is the amount you need to pay each year before your plan starts paying benefits. What is co-insurance? Patient amount of share based on % of charge that Insurance company accepts. Co- insurance is paid after plan's deductible is met. Who do you call if the GHC is closed? Call Emergency Mobile Crisis (211) for behavioral health concerns 911 for Medical concerns
