Inspection Results Following recent inspections, the 33 practices within the CCG footprint were assessed as follows: 6 as “Outstanding” 27 as “Good”

10 Key Areas of Focus for 2018/19 85% of patients are treated within 62 days of urgent cancer referral 95% of patients are waiting less then 4 hours when attending A&E Reduce delays for patients waiting to leave hospital <18 weeks between GP referral to hospital treatment, all specialties No-one waits longer than 52 weeks from referral to treatment No-one waits longer than 12 weeks for Children and Young People’s Emotional and Wellbeing Mental Health Service treatments

26 Queensway Court Large Extra Care Housing facility in south Leamington A resident identified high levels of ‘health and care’ activity and asked the question ‘Could this be done more efficiently?’ Established a small cross system group including the resident and manager – and all agreed the answer was ‘Yes!’ Initial 6 month pilot established in November 2017 (now extended to 12 months)

28 Oken Project Priory Medical Centre was approached by The Thomas Oken and Nicholas Effyler charity Wanted to offer a proactive care service, over and above core NHS services 46 hours per week of Advanced Nurse Practitioner time to support local GP practices to deliver proactive care to residents of Park View and housebound patients

Project confirmed until (3 years in total) Outcomes to date have been very positive: A lot of learning from the project Feedback from all parties has been excellent Communication – team referred to as the Oken Nurses KPIs all moving in the right direction The Oken Project Team … Both Queensway Court and the Oken Project are ‘pilots’ for the Out of Hospital Programme

31 Background SWFT awarded a 3 year contract for Out of Hospital services across Warwickshire, including a 2 year transformation programme; CWPT Lead Provider for Out of Hospital Programme across Coventry; The core purpose of the transformation programme is to configure services to keep people healthy and well at home, for as long as possible.
