Care Coordination in Australia Reasons for Use Package A resource health and welfare workers can use to facilitate a therapeutic conversation with clients about their dual diagnosis issues which aims to assist treatment planning. Designed by Simon Kroes and Kevan Myers of Nexus Definitions abound…. Care coordination is a client-centred, assessment-based interdisciplinary approach to integrating health care and social support in which an individuals needs and preferences are assessed, a comprehensive care plan is developed, and services are managed and monitored by an identified care coordinator following evidence-based standards of care (Brown 2009). Evidence of positive outcomes (Rosenburg & Hickie 2013) Reduction of duplicated service Reduced hospitalisation Increased social connectedness

4 What is it? “A simple tool for complex conversations about dual diagnosis” The Reasons for Use Package is a compilation of existing tools, interventions and information that may be used following the administration of the RFU scale. Augments existing skills

The package has several benefits, including but not limited to: Recovery based and consumer friendly Assists treatment planning Increases confidence and knowledge around dual diagnosis interventions in a straight forward manner Highly cost effective to train and use the resource Can be used by a wide variety of services and professionals Aligns with current State and National mental health and alcohol and other drugs strategy
