A resource health and welfare workers can use to facilitate a therapeutic conversation with clients about their dual diagnosis issues which aims to assist treatment planning.

Definitions abound….
Care coordination is a client-centred, assessment-based interdisciplinary approach to integrating health care and social support in which an individuals needs and preferences are assessed, a comprehensive care plan is developed, and services are managed and monitored by an identified care coordinator following evidence-based standards of care (Brown 2009).
