Just like a survey form…
Questions
Forms are a list of questions that users in the field complete. Once submitted, we call them form responses.
So, a form is in essence a collection of questions
In case of single or multiple choice style questions, option sets are needed for users to select from.

Building the form by creating new questions or adding existing questions
Associating option sets, when needed, by creating new option sets or importing existing option sets

5. Creating new forms To create a new form, first select Forms
Then select Create Form

6. Creating new forms Give the form a logical name
Name of the form shouldn't include special character.
Select More settings
Choose whether the form will allow for SMS, or texts, as form responses

Choose whether the form will allow observers to submit incomplete responses (missing answers for required questions)
Select Save to create the form and save changes

9. Creating and adding questions to forms
After a new form is created, we need to add questions to it.
There are two ways of doing this:
Create a new question and add it to the form OR
Add an existing question to the form

To create or add a question
Select Forms
Then select a form to edit or create a new one

Next, select Add Questions

Click Create Question to create a new question to add Or
Mark the boxes next to existing questions and click Add Selected Questions to Form

13. Creating questions to forms
To create a new question to the form, enter the following information:
Code: Provide a 2-20-character codename for the question. This code saves space when reports are generated on ELMO. It can also serve as a codebook glossary item (for researchable datasets)
Type: Choose one of question types. This will be covered in detail in the next section.

Title: type the text of the question itself.
Hint: type any instructions the field user may need to answer the question.

Is Key Question?: If this box is marked, it will add a column showing these answers when viewing all the Responses in the Reponses view.
Condition: The question will only appear if a certain answer is given (or condition is met) on an earlier question in the form.

Hidden: If this box is marked, the question will not appear at all when submitting a response to the form. This allows the easy reuse of master templates when not all questions apply to the situation
Required: If this box is marked, the question must be answered before an observer can submit a response, unless the observer enters a special override code.
Select Save to create the question and save changes

17. Changing the order of questions
Once questions are added, the order of questions in the form can be adjusted by selecting, dragging, and dropping the question.

The next slides will provide an overview to each of the 16 question types
Each slide provides:
a description of the question type alongside
an image of how the question appears to an observer while completing a form via ODK Collect

20. 1. Text questions SMS Compatible
For Text questions, the user enters a single line of text

21. 2. Integer questions SMS Compatible
For Integer questions, the user enters a whole number
This example has been set to restrict answers to values between 0 and 10 with an accompanying hint to describe the restriction

22. 3. Decimal questions SMS Compatible
For Decimal questions, the user enters a number containing decimals
This example has been set to restrict answers to values between 0 and 1 with an accompanying hint to describe the restriction

23. Setting restrictions on integer/decimal questions
Integer and Decimal questions allow for minimum/maximum restrictions
Marking Strictly greater/less than will prevent the form from accepting values equal to or greater than/less than the value entered

24. 4. “Select one” questions
For Select One questions, the user selects one option from a list of given options
Options for Select One questions are displayed as radio boxes in ODK Collect

25. 5. “Select multiple” questions
For Select Multiple questions, the user can select one or more options from a list of given options
The options for Select Multiple questions are displayed as checkboxes in ODK Collect

For Date + Time questions, the user selects a date using the calendar feature and a time using the clock feature
Date questions include only the calendar for selection
Time questions include only the clock

Long Text questions look the same as text questions, but are intended for paragraph-length answers

28. 10. Location questions NOT SMS Compatible
For Location questions, the user taps Record Location to use GPS software to obtain the user’s current coordinates
Tip 1: retrieve GPS coordinates while outside (a good hint to add for this kind of ELMO question!)
Tip 2: if it doesn’t automatically resolve in a couple of minutes, just record. It will make its best guess!

For Image questions, the user takes a picture or chooses a jpg or png image

For Annotated Image questions, the user takes or chooses a picture and annotate it

For Signature questions, the user writes one’s signature

For Sketch questions, the user sketches an image

For Video questions, the user records or chooses a video file
As for image, video, or audio submission, please be reminded of that submitting a big size of file may cost a lot of data.

35. Conditional Questions
For example, conditional question If ‘other’, please describe will only appear when the answer for the previous question includes Other
Conditional questions will appear in the form with the image of chain and number of the question it is conditioned

36. Adding a Question Group
Click Add Group to make a set of questions appear on the same page when viewing on the table.
Once it is clicked, new fields under Add Group will appear.

37. Adding a Question Group
Name: In this space, type the question itself.
Hint: In this space, type any instructions the field user may need to answer the question.

38. Adding a Question Group
Check Repeatable? box, to enable multiple instances of the same response for this group of questions
(ex. When an observer need to interview multiple interviewees for the same questions)
Select Save to create the group and save changes

39. Adding a Question Group
Once a group is added, it will appear with the folder image in front of the name of group.

40. Adding a Question Group
Questions can be grouped by clicking, dragging, and dropping the question into the group
Questions put into the same group will look like this

41. Making a Question Grid
You can present a group of similar questions together as a “grid”

42. Making a Question Grid Grids can be created when
Questions are in the same group
Questions are ‘Select One’ questions
Questions have the same option set.
For example, question Chopstick, Fork, Knife, Spoon and Others: -belongs to the same group -are ‘Select one’ questions -share same option sets, ‘Yes/ No’

Remember, for all question groups, observers don’t have to swipe to answer each question, but ”scroll down” to view all questions in the group on the same page.
