2. Problem Statement
Primary care within South Western Sydney is characterised by individual clinicians providing a series of short episodic consults across independent practices and clinics. This is not conducive to systematically providing patient-centred, integrated care to patients with chronic conditions. Furthermore, poor data quality and poor communication connectivity between health providers does not support coordinated patient care.

3. Core Aims and Objective
The Enabler’s Project is part of SWSPHN’s role in improving integrated health care across South Western Sydney. SWSPHN will scope, develop and implement a coordinated support program that will assist general practices to better integrate with other service providers and enable them to deliver best practice quality healthcare through:
Linking with a formal clinical quality improvement program for general practices (including data extraction, benchmarking and clinical audits)
Practice capacity development (including secure messaging, e-referral systems, My Health Record).
Training in patient centred care to prepare practices for future integrated health strategies and care coordination
To enable practices in South Western Sydney with the tools necessary for more integrated healthcare, and to prepare them for models such as Health Care Homes.
