2 Failed Identity Proofing Messaging

3 Request Proof of Identity…
Request proof of identity from the approved list on the slide (slide 4) If proof is provided proceed with manually verifying identity If proof is not provided, DO NOT proceed with manually verifying identity until documentation is provided Explain to the individual that they have failed to pass the identity proofing service required by federal law. The customer must submit documentation on the Washington Healthplanfinder by mail, online, or in-person with an assister Initial applications: The application will be purged in a batch job, if not completed, within 90 days as an Incomplete Application WAH Renewals: The household will be dis-enrolled after 60 days of non-completion

4 Approved Documents for ID Proofing
Primary Documents (List A) with picture document. Please note that the following documents are acceptable if expired as long as they contain the individuals name, picture and date of birth: · U.S. passport or Passport card · School identification card · U.S. State or territory issued driver’s license or ID card · Certificate of Naturalization (N-550/N-570) · Certificate of Citizenship (N-560/N-561) · Voter registration card · U.S. Military card or draft card · Permanent Resident card or Alien Registration card · Employment Authorization document (Form 1-766) that contains a photograph · U.S. Coast Guard Merchant Mariner card · Foreign Passport that contains stamp or Machine-readable Visa with stamp · Foreign Passport with Form i-94 or i-94A · Passport from the Federated States of Micronesia (FSM) or Marshall Islands (RMI) with Form i-94A · Document from federally recognized Indian tribe that includes your name and the name of the federally recognized Indian tribe that issued the document, and shows your membership, enrollment, or affiliation with the tribe. Documents you can provide include: à A tribal enrollment card à A Certificate of Degree of Indian Blood à A tribal census document à Documents on tribal letterhead signed by a tribal official Secondary Document (List B). If customer submits a primary document with no picture ID, then the customer must also submit a copy of a second document (two items total): · Military dependent identification card · American Indian Tribal document · Birth certificate · Social Security card · Marriage certificate · Divorce decree · Employer identification card · High school or college diploma · Property deed or title

5 Documentation Upload Instructions
A copy of the individual’s identification must be uploaded to the individuals Washington Healthplanfinder account before manual ID proofing can be performed Once on the customers dashboard navigate to the Document Center tab Select Upload One Now Select Document Category Proof of Identity Provide the following comments in the additional information: Individual failed ID proofing, submitted proof of ID. ID confirmed update document to “reviewed” Click Submit to upload document

After identity has been confirmed and documentation has been uploaded – brokers/navigators can update the customers verification status of identity by: Access the individuals account home Click Verify ID Proofing under Quick Links Washington Healthplanfinder will route you to the Application Review screen

4. You will see messaging here stating “We were unable to verify items in red. Go to Action Center from your account to upload necessary documents”. You can Scroll down to see the red areas unverified and will find the Primary Applicant name, DOB, SSN, etc. highlighted in red 5. Click the green ID Proofing link to manually verify their identity

6. Select Manually Verified from the drop down and enter current date in the Verification Date Field 7. Click Update 8. On the bottom of the Application Review screen select back to get back to the customers account home

9 Complete the Application
Once you have manually ID proofed the primary applicant you can Complete My Application from the Quick Links From here you will be taken to the Contact Information screen and are able to complete your customers application and enrollment
