The form has the same layout – for each grade columns 1 (Pertussis vaccine completed) +2 (PME) +3 (PBE) must equal column 4, total enrollment.

19 LHD Administrative Portal
Type in your User Name. Type in your Password. Note: Both are case-sensitive. Click the Log in button. One website you’ll use starting this spring to update school lists and this fall to identify delinquent schools/centers is the administrative portal. You each will receive by if you haven’t already, your user name and password. On the login page is a step by step powerpoint for you to view later on the important features you’ll need.

Successful login will bring you to this screen. Once you login, you’ll see this screen.

21 Accessing Kindergarten Delinquent Lists
Click on the Generate List of Schools not reported hyperlink (circled) Then select, select Generate list of Schools Not reported

22 Your county-specific delinquent list appears.
Click the Export to CSV button (circled) to export the list. Similar to the child care delinquent list, you select export to CSV

23 When the File Download pop-up opens and asks whether “to open or save this file?”, select Save.

24 Set the location (circled) and file name (arrow) for the saved file
Set the location (circled) and file name (arrow) for the saved file. The default file name is Output, but you can change it to K Delinq List. Set the location and file name

25 Keep the file type as Microsoft Excel Comma Separated Value (circled).
Click the Save button (arrow) to save the county delinquent list. Keep the file type as a MS Excel comma separated value, and select Save

26 Once the download is complete, click the Open button.

27 This file contains the most recent list of non-reporting Kindergarten schools in your county.
The file contains: CoCode (county code), DistCode (district code), SchCode (school code) CoName (county name), DistName (district name), SchName (school name) PhysStreet (phys. street), PhysCity (physical city), PhysZip (physical zip) MailStreet (mailing street), MailCity (mailing city), MailZip (mailing zip) AC (area code), Phone (phone number) Pub/Priv (public or private) Rep (Field Representative) ( address). Columns borders (red) can be expanded to reveal column information. To do so, double click on the ‘right’ column border and column will auto-expand. The file contains the most recent list of non-reporting kindergarten schools in your county.

Select: Generate List of Schools Show Submission Status (circled) The 7th grade admin is in the making and will be similar to the kindergarten admin.
