6. What does it mean? ADVANTAGES:
All providers and commissioners in an economy work together
Ends the provider / commissioner split
Opportunity to reform the transactional basis of NHS contracts
Binds an economy to common aims and health outcomes
Has potential to place patient in the centre of care delivery
Can fix costs and ensure more money goes to delivery of care
ISSUES:
Accountability is currently less clear
Structures not defined
Unclear who controls the process
How will this be delivered?

7. What could services look like?
All providers in a position to deliver against defined health outcomes
Discussion about primary care delivering services currently delivered in acute sector at a price differential less relevant
Cross-tier services delivered in partnership – right service at the right time in the right environment
May see co-location of services, especially primary care based services

8. What will provision look like?
Extended primary care services and community monitoring services – PEC delivered must be within the ACO / ACS / ICO
Not just about traditional ophthalmic care. What else can be delivered? E.G. Healthy Living Service in Dudley
Need to define and understand where optometry fits in wider care pathways, i.e. The role in elderly care, falls, dementia, etc.
Need to understand how services can be delivered in a new contract mechanism and in partnership.
Need to define role of primary care contracting and where/if it fits with an ACO/S structure.
