Open Enrollment & HEM Health Assessment Actions Required: Action 1: PEBB Open Enrollment Completion – confirm your benefit elections, make changes, re-enroll in flexible spending accounts, AND indicate your HEM participation status for the 2015 plan year.  Action 2: HEM Health Assessment Completion – If you elect to participate in the HEM for 2015, you and your enrolled spouse/domestic partner must complete the HEM Health Assessment again between September 1 and October 31, 2014. November 8, 2018

Vision Coverage No longer attached to a medical plan May elect to “Enroll” or “Not in Enroll” Employees opting out of medical can now have vision coverage “Federal” employees will now have vision coverage through PEBB Does not provide “cash back” if elect not to enroll in coverage Commuter Flexible Spending Accounts Commuter Transportation – max. is $130/month for bus, train, van pooling Parking Account – max. is $250/month Parking expenses near employer premises or location to commute to work using mass transit, car pool, van pool, etc. OSU parking fees deducted from payroll is already pretax (cannot be included as monthly expense/contribution to this account, but must be considered in the allowed maximum) November 8, 2018
