2 BENEFITS OPEN ENROLLMENT
Welcome to Open Enrollment! Elections made during open enrollment will become effective August 1, 2017 Meeting Street Schools offers you and your eligible family members a comprehensive and valuable benefits program. We encourage you to take the time to educate yourself about our options and choose the best coverage for you and your family. Medical & Rx Dental Vison Life & Disability Voluntary Life Annual Enrollment gives you the opportunity to enroll in benefits that help provide security for you and your family by protecting your health and lifestyle. Your benefits represent a significant portion of your total compensation; for this reason AMBFOB wants to ensure that every employee has a clear understanding of the benefits offered. During our annual enrollment period, Benefit Counselors will provide benefits education and enrollment services by phone, and all employees are required to call the Benefits Enrollment Center in order to maintain benefits for Benefit counselors will be available to review your current benefits, confirm your 2016 elections and answer any questions you may have. Before calling the Benefits Enrollment Center, make sure that you have dates of birth and social security numbers for all dependents to be covered – and plan for your enrollment call to take approximately minutes. 3 Making Benefit Changes
Unless you have a qualified change in status, you cannot make changes to the benefits you elect until the next open enrollment period if your deductions are pre-tax. Qualified changes in status include: Marriage Divorce Legal separation Status change Birth or adoption of a child Change in child’s dependent status Death of a spouse, child or other qualifying dependent Change in residence due to an employment transfer for you or your spouse Commencement or termination of adoption proceedings Change in spouse’s benefits or employment status Remind that qualified life events outside of Open Enrollment must be done within 31 days of the event. 4 Who is Eligible? If you are a full-time employee (working 30 or more hours per week), you are eligible to enroll in the benefits described in this presentation. The following family members are eligible for the following through : Legally married spouse Children to age 26 Disabled children November 7, 2018

6 Telemedicine – Blue CareOnDemand
The cost to a member on a High Deductible Health Plan will be $59 and will apply to both the deductible and Maximum Out-of-Pocket (MOOP).  The cost to a member on the PPO Copay plan will be $20 Primary Care Copay. Download the Blue CareOnDemand app on a smartphone or tablet via Google Play or the App Store or by going to on a computer and registering using their My Health Toolkit address and password. There is not a phone number to call to register. You’ll need the employee's member ID to complete the online profile. Only enrolled members (including dependents) will have access to this resource. There will be a claim that is filed under that member ID. The claim may take 1-2 weeks to be reflected in the member’s records on My Health Toolkit.  Payment information can be entered at time of use or saved on the Blue CareOnDemand website for ease of future use. Blue CareOnDemand is BlueChoice’s telemedicine. Access board certified doctors 365/24/7 Available to enrolled employees and their dependents only If online doctor can’t diagnose or no prescription is written, the member will not be charged for the visit

9 Health Savings Account (HSA)
Must be enrolled in a high deductible health plan (HDHP) Tax exempt account owned by the individual Used to pay for qualified medical expenses 2017 IRS Maximum Contributions $3,400 Individual / $6,750 Family $1,000 catch-up contribution for age 55+ Account must be with Optum Bank to make payroll deducted pre-tax contributions Meeting Street Schools will contribute $800 for employee only and $1,600 for all other coverage tiers to the HSA account. (Annual amounts to be paid into accounts ½ August and ½ in February.) Not eligible for HSA tax deduction if enrolled in Medicare No OTC medication reimbursement without prescription Why consider an HSA? -As an HSA user, you save in several ways such as lower monthly health insurance premiums & a triple tax advantage on your HSA contribution. (Savings for your future) -With an HSA, you own the account and all contributions. Your HSA balance rolls over and remains available for qualified expenses even if you change health plans, retire or leave the company. Once you reach a determined account balance, you can invest your HSA funds for increased earning potential. (You Don’t Have to Change Doctors) -Your HSA-qualified health plan gives you access to the same in-network doctors as the traditional plans offered currently. 11 Dental Benefits There are no plan or cost changes to dental benefits.
Meeting Street School’s plan allows you to seek treatment from the dentist of your choice. 14 Disability Income Benefits
Meeting Street Schools provides full-time employees with short- and long-term disability income benefits, and pays the full cost of this coverage. In the event that you become disabled from a non-work-related injury or sickness, disability income benefits are provided as a source of income. Short-Term Disability (STD) Long-Term Disability (LTD) Benefits Begin 8th day for Accident or Illness 91st day Maximum Period of Payment 12 weeks SSNRA (Social Security Normal Retirement Age) Percentage of Income Replaced 60% 66 2/3% Maximum Benefit $1,500 per week $7,500 per month November 7, 2018

15 Basic Life Insurance Meeting Street Schools provides full-time employees with group life and accidental death and dismemberment (AD&D) insurance, and pays the full cost of this benefit. 100% of base salary to a maximum of $300,000 Guarantee Issue of $155,000 Conversion and Portability Age Reduction – Reduced to 50% when the employees attains the age of 70 November 7, 2018

16 Voluntary Life Benefits
Employees may purchase supplemental life insurance. When you enroll yourself and/or dependents, you pay the full cost through weekly payroll deductions. Employee: $10,000 increments to a maximum of $350,000. Guaranteed issue $50,000 for newly eligible employees Spouse: $5,000 increments to a maximum of $250,000, not to exceed 100% of employee’s benefit Guaranteed issue $25,000 for newly eligible Dependent Children: $10,000 November 7, 2018
