Guarantee Issue for employee up to $350k during new hire enrollment Coverage amounts for employee range from $10k - $500K (increments of $10k) Coverage amounts for spouse range from $5k - $250k (increments of $5k with a Guarantee Issue of $30k) Coverage amounts for children range from $2.5k - $10k Employee paid, post tax

9 Lincoln Voluntary Short Term Disability
You are able to elect a weekly benefit in $50 increments, up to a maximum of $2,500 per week, not to exceed 60% of your salary Two options to select from - a 14 day wait period with slightly higher rates or a 30 day wait period Benefits reduced by income received by state disability plan Employee paid, post tax

10 Lincoln Voluntary Long Term Disability
Monthly benefit of up to $5,000, not to exceed 50% of your monthly earnings Benefits begin after a 90 day elimination period Employee paid, post tax

11 UNUM Individual Long Term Disability
Up to $7, per month guarantee issue (no medical exam required) Offset by group LTD coverage, whether on not elected Discounted premiums that will not increase – guaranteed renewable, non-cancellable converage You own the policy – take it with you at the same discounted rate Employee paid, post tax

12 Flexible Spending Accounts (FSA’s)
Health Care Account: up to $2,500 per year in pre-tax contributions to cover out-of-pocket health care expenses for you and dependents Dependent Day Care Account: up to $5,000 per year to cover out-of-pocket dependent day care expenses Business Expense Reimbursement Account: contracted amount that is pro-rated based on calendar year for new hires

13 Health Savings Accounts (HSA’s)
You must be enrolled in an HSA eligible medical plan offered by your company to enroll in a Health Savings Account Annual Contribution Maximums: $3,350 (single) $6,650 (family) $1,000 Catch-Up (age 55 or older) Account is with Optum Health Bank Contribution amount may be changed at any time

Coverage is available for you and your eligible family members in amounts from $5,000 up to $300,000 There is a long-term care benefit built in to the universal life plan, as well as a death benefit restoration feature Critical Illness with Cancer Insurance This coverage offers a lump-sum benefit payment when first diagnosed with a covered critical illness which include: Invasive cancer – Stroke - Heart Attack - Major Organ Transplant - Renal Failure – Paralysis – Blindness - Lou Gehrig’s Disease

18 Schwab Retirement 401k Savings Plan: 25% max contribution per paycheck up to $18,000 annually 401(k) Catch-Up Provision: 25% max contribution per paycheck up to $6,000 annually, must be age 50+ Elective Deferral Plan: 20% max contribution per paycheck up to $35,000 annually upon completing eligibility of 1,000 hours within first 12 months or medical directors employed for 6 months EDP is a one time irrevocable election

19 Schwab Retirement The total max contribution an employee and employer are allowed in 2015 is $53,000 (excluding the $6,000 catch-up provision) The plan does not match the 401(k) deferral because of the ability to defer the additional $35,000 as EDP funds $18, (k) + $35,000 – Elective Deferral Plan $53,000 max contribution per IRS (2015)

20 Workers’ Compensation
If an injury occurs while “on the job”, Employees should immediately: Contact their Medical Director Complete the “First Report of Injury Form” within 24 hours. The completed form should be forwarded to your Payroll/Benefits coordinator.
