2. 139R ready for signature by Enrollment Officer (Part 1-5).
Ink signatures are required.

3. Original birth certificate and social security card submitted to SSG Shehan. Copies will be made with original returned.

4. Most recent passing APFT (height and weight, if applicable).

5. Your instructor must recommend your enrollment to SSG Shehan.
