Triple Aim: Improve the health of a defined population Enhance the patient care experience Reduce the cost of care Definition – Integrated Delivery System: Managing the entire spectrum of patients you serve to keep them as healthy as possible and manage their care needs in the lowest intensity setting possible, and taking financial accountability for total population health care costs and measured outcomes.
