3 Data File Validation Emails
– 1 Business Day after upload Thank you for submitting your data file to NACOR for processing. Your file successfully passed validation. Data analytics will be completed and reports available in the NACOR Dashboard within 24 hours. 19 of 20 records (95.0%) were valid for submission in the file: NACOR_SAMPLE_PID(004)_DOSSTART( )_DOSEND( ).xml. This validation checks whether your file adheres to the NACOR XML schema and performs certain basic logic checks. It does not confirm accuracy of the data submitted or performance relative to CMS QCDR/QR programs. To review accuracy of the data provided and, if applicable, whether your data meet QCDR/QR performance and reporting thresholds, please review the data and CMS Quality Reporting reports within NACOR. Below is a summary of any issues: • AnesthesiaStartTime must be greater than AdmissionDate (occurs {x} time(s)). The records with the following AnesthesiaRecordIds are affected: {List of Record ID’s} Additional information and support resources: NACOR Data Submission Guide | NACOR Support | AQI Developer Website | NACOR Reporting Portal This communication is confidential and may be privileged or otherwise protected from disclosure and may include proprietary information. Unauthorized reproduction or disclosure of this information in whole or in part is prohibited. Copyright © 2017 ePreop, Inc. Your file failed validation and will not be incorporated into NACOR. Please see the full validation error log on the NACOR Data Submission Portal which identified failed cases. You may fix errors for those cases and resubmit your file to NACOR. © 2016 AMERICAN SOCIETY OF ANESTHESIOLOGISTS.

4 Common Data File Errors: File Naming Convention
Required identifiers for proper formatting: PID – Your AQI Practice ID number must have three or four characters. If your practice ID is less than three characters, use preceding zero’s “0” in the Practice ID to complete the file name. .xml – This is the file extension Recommended identifiers: Practice name (or abbreviation) DOSSTART – The first date of service in your data file DOSEND – The last date of service in your data file Example – Production File: ASA Anesthesia_PID(003)_DOSSTART( )_DOSEND( ).xml © 2017 ANESTHESIA QUALITY INSTITUTE

5 Common Data File Errors: Email Notification Set Type
Notification Set Type was added to the 2017 xml schema. If you receive this error message you need to add the following to your data file: " NotificationFirstName" " NotificationLastName" " NotificationAddress" The address that is listed in this field will receive the file validation feedback s. © 2017 ANESTHESIA QUALITY INSTITUTE.

6 Additional File Submission requirements
Zip files are not allowed 150 MB File submission limitations Ensure that you have whitelisted for validation s Ensure that practice identifiers are 3 or 4 characters. If you practice has a 2 digit number, ensure that you include the leading 0 to make it 3 characters. This is relevant in the file naming convention and within the xml node for the practice. 7 Deleting and Replacing a File
AQI has made deleting and reloading data easier. The 2017 XML updates include a file version “AQIXMLVersion” which have two possible values “2017V1.0” and “2017V1.0R.” 2017V1.0 – This value will tell ArborMetrix to process the file as normal inserting data into NACOR. This operation performs insert / update functions. 2017V1.0R – This value will tell ArborMetrix to delete and replace all data in NACOR with matching AnesthesiaRecordIDs. This operation performs delete / insert functions. For more information review the 2017 NACOR XML Schema on the AQI Website: © 2017 ANESTHESIA QUALITY INSTITUTE

8 Reporting Central Line Measures – MIPS 76 and AQI 32
MIPS #76: Prevention of Central Venous Catheter (CVC)-Related Bloodstream Infections AQI #32: Composite Procedural Safety for Central Line Placement Reporting either one of these measures is intended for clinicians who attempt central venous cannulation insertions. As paraphrased from the ASA CROSSWALK (p. vi of 2017 edition): Anesthesiologists may report codes for diagnostic or therapeutic procedures in addition to the anesthesia code for the primary operative procedure. When an anesthesia provider administers anesthesia in support of procedures like an insertion of a central venous access device, the appropriate anesthesia code should be reported. Anesthesiologist places the line, the surgical CPT should be placed in the <CPTValue> section of the xml data file.  Anesthesiologist provides anesthesia for the line placement, the anesthesia CPT code should be placed in the <CPTAnesValue>. © 2014 AMERICAN SOCIETY OF ANESTHESIOLOGISTS.

9 2018 Schema Changes 2018 Schema Changes
Added comorbidities (ICD 10 CM codes + Registry Codes) Added capability to report Internal Improvement Measures (IIM) Include retired ASA/AQI measures, retired PQRS/MIPS measures, measures in development, other measures as identified. 2018 schema versions 2018V1.0, 2018V1.0R Schema changes: This list is not a comprehensive list, the final list will be posted early December 2017 and implemented Spring of 2018 – date to be determined. Vendor Agreement: Vendor agrees to participate in the Registry on behalf of each Participant by transmitting data through a secure site designated by AQI for the collection and submission of data pertaining to the practice of anesthesiology. Vendor agrees to submit data in accordance with the AQI Data Specifications. Vendor understands that the data elements may differ depending on the Participant’s desired level of reporting (e.g. reporting data to a Centers for Medicare and Medicaid Services program). Vendor further understands that the AQI Data Specifications will evolve over time and AQI will provide reasonable notice to Vendor of any material changes. © 2014 AMERICAN SOCIETY OF ANESTHESIOLOGISTS.

10 Merging/Formatting Services
As of July 2017 AQI no longer merges data files. All files must be submitted merged and in xml format. Services are available for practices if they need merging, formatting or both Practices must be identified by 12/15/17 s will be sent to practices who required merging services in 2016 Practice needs to schedule a call with AQI and ePreop to have a call scheduled © 2017 ANESTHESIA QUALITY INSTITUTE
