Figure 2. A nomogram relating total body weight, height, and gender to the body weight that should be used to calculate dose from recommendations that are scaled to total body weight. The dots show ideal body weight, as in Figure 1. The scaling between Figure 1and Figure 2assumes that published dosing recommendations scaled to total body weight are exactly correct for persons of ideal body weight, which accounts for the finding that the dots line up on the line of identity.
