CARD 1: Friendly KIA / WIA

(2) Where is the deceased individual? T: Time. When did the death occur? A: Actions: (1) Actions taken by unit/agency. (2) Assets/support needed from BCT? (3) Did the unit return fire? If so, are there enemy WIA/KIA? What is the BDA? 1. Unit conducts immediate first aid and react to contact drills 3a. CARD 3: Security Force/Host Nation Soldier/Police Death
One or more of the following are killed 1. US / Coalition Soldier 2. DOD Civilian / coalition civilian 3. Any civilian wounded by US forces 1. Unit conducts immediate first aid. medical assets, and not coalition medical assets, should treat victims. Unit should only request BCT medical assets when all civilian assets have been exhausted. 3a. Notify / Staff Action Checklist: S3 / BDE CDR / XO / CSM S2: Estimate of who the attacker(s) were and why they chose the target. Collects facts and conducts pattern analysis S3 Air/ALO: Air tasking to provide aerial support to incident area if required by responding US units. BDE Surgeon: Responsible for tracking civilian deceased. SJA: Legal issues with providing support to Host Nation officials IO/PSYOPS: Tactical Humit Team (THT) support to determine the perpetrator PAO: Prepares statements BCT LNO: Notify DMAIN and PAO BN LNO: Collects information/details of incident If required, Battle Captain notifies one or more of the following: Incident involving local populace: CMO Ministry of Interior MCS-L operator zooms in on incident, displays only the applicable overlays, and checks Blue Force Tracker Text messages from TFs and/or DIV Alert all BN TOCs if upgrade FPCON is necessary 2a. CARD 5: Non-Lethal Fire Fratricide
Non-lethal fire fratricide is the distribution of conflicting information between the source and its higher headquarters FLASH TRAFFIC! 1. Individual identifying the conflicting information immediately stops the information flow. 2a. SPOT report to BCT TOC: Battle CPT / Battle NCO gathers info: S: Size (1) What was the information? (2) How much of the incorrect message was transmitted? (3) Who is the proponent of the correct information? (4) Who is the source of the incorrect information? A: Activity (1) What medium was the message transmitted? L: Location (8-digit grid) (1) Where was the information transmitted (2) how many people received the incorrect information? T: Time. (1) When did the information travel? (2) How many times was it transmitted? A: Actions: (1) Actions taken by unit/agency. (2) Assets/support needed from BCT? 2. Unit notifies BCT TOC 3a. CARD 6 Homicide / Suicide Bomber Attack (Dismounted/Vehicular)
Explosion Occurs 1a. Immediate actions by unit: 1) Secure the area: -Cordon off site within a 200m radius -Control all individuals entering and exiting the site -Establish FCP 2) Report to BCT TOC 3) Notify Police Services, security forces, and local fire-fighters 4) Establish a LUP for BCT Assets (EOD Tm) 5) Take Digital Photos of effected area 1. Unit responds to explosive incident. FLASH TRAFFIC! 2. BN TOC sends report to BCT TOC 3a. T: Time (1) What time did the detonation occur? A: Actions: (1) Actions taken by unit. (2) Assets/support needed from BCT? (3) If explosion occurred along a route, does the unit need to close the route? 3. Battle Captain begins notification procedures. Submits report to Div within 30min. 4. CARD 7: Missing Soldier/Contractor (POW/MIA/AWOL)
Unit missing Soldier 1. Unit identifies Soldier is missing FLASH TRAFFIC! 1a. Immediate actions by unit:: (1) Unit conducts local search to find missing Soldier (2) Notify higher HQs. (3) If on FOB: (a) Stop all non-mission-essential outgoing traffic (b) Conduct FOB wide search (c) Inspect all outgoing vehicles (4) If outside FOB: (a) Secure area (b) Conduct thorough search of area 3a. Notify / Staff Action Checklist: S3 / BCT CDR / XO / CSM S1: Family / Rear Detachment notification. S2: Enemy situation in area that might indicate hostile action. S5: Coordination with civil leaders in vicinity of disappearance. S3 Air: Availability of Army aircraft for search. ALO: Availability of Air Component assets for search/CSAR. Chaplain: Provide religious support to family member(s) of missing/captured personnel. Process the captured, missing, or detained personnel upon return to unit. PAO: Flash PA response. 2. Unit reports to BCT immediately 3. Battle CPT begins notification procedures, submits report within 30 minutes 2a. SPOT report to BCT TOC: Battle CPT / Battle NCO gathers info: S: Size (1) How many Soldiers are missing and from what unit? A: Activity (1) Who is the first line supervisor and where/when was the Soldier last seen? (2) What were other individuals in the area doing before, during, and and after unit discovered the Soldier was missing? (3) Was there any threat activity in the area that may be responsible for the disappearance? L: Location Soldier was last seen T: Time. - Time Soldier was discovered missing A: Actions taken by unit. -Conducting lock down and initial investigation 4. Battle NCO initiates separate 1594 Log to track actions for finding the missing Soldier 5. Unit submits follow-up reports to BCT, BCT submits reports to Div

8 CARD 8: Detainee Unit captures detainee 1. Unit conducts immediate actions and sends report to BCT 1a. Immediate actions by unit:: (1) Secure (2) Silence (3) Segregate (4) Safeguard (5) Speed (to detainee facility) 2. Place detainee in unit holding area until assets are available to transport detainee to BCT detainee facility at FOB 3a. Notify / Staff Action Checklist: S3 / BDE CDR / XO / CSM S2: Initial information requirements IO/PSYOPS: Capture events for messages that support the secure and stable environment SJA: Advise unit if troops are involved in detaining perpetrator PMO: Ensure division holding facility is coordinating with detaining unit for receipt of detainee. 2a. 9 FLASH TRAFFIC! CARD 9: Downed Aircraft Aircraft goes down
1. Unit reports to BCT TOC. If DMAIN contacts BCT, then BCT immediately contacts closest unit FLASH TRAFFIC! 1a. Immediate Unit Actions: (1) Unit secures area, establishes outer and inner cordon (2) Initiate first aid to injured personnel (3) Request MEDEVAC CARD 11 (4) Determine cause of downed aircraft 2b. Notify / Staff Action Checklist: S1: Replacements, escorts and documents processing. S2: Trend / pattern analysis to recreate event graphically to include: Requests imagery products of crash site Directs ACE to conduct threat assessment Pull ISOPREP cards on all PDB S3 Air: Coordinates for aircraft for quick reaction. ALO: Availability of Air Component assets for surveillance/CAS/CSAR, pass type and location of enemy weapon that downed the aircraft to ASOC. S4: Property destroyed, floats, adjustment documents. S5: Ensure MSC teams gather info. PAO: Initiates flash PA traffic. SJA: Provide assistance to Rear elements. IG: BPT 15-6 if required 2. Battle Captain begins notification procedures. 11 CARD 11: Power Outage Power outage reported
1. Unit conducts immediate actions and sends report to BCT 1a. Immediate actions by unit: (1) Contact local power facility supervisor (2) Determine source of power failure (3) Report to BCT TOC (4) If power can not be restored immediately, request assistance from BCT 2. Minister of public works and CA determine the best method to restore power if the power facility can not repair itself. 2a. SPOT report to BCT TOC: Battle CPT / Battle NCO gathers info: S: Size (1) How large is the physical area effected by the power loss (2) Size of population effected A: Activity (1) What was the cause of the power outage? L: Location (1) What areas are effected—rural and urban. T: Time. (1) What time did the power go out? (2) How many times has this happened in the last days? (3) Once the power is restored, determine the total time the power was out. A: Actions taken by unit. (1) Assist local power plant supervisor to repair (2) Determine cause of failure 3a. Notify / Staff Action Checklist: S3 S2: Potential threat actions / sabotage. S4: Services provided. S5: Critical civil services. Eng: Power grid repairs. PMO: Crowd control, police, security, and looting control. 3. Battle Captain begins notification procedures, submits report within 30 minutes 4. Requests assets from local nationals and/or DIVENG to fix power loss and coordinates link up with unit in sector 5. Once power is restored, unit submits follow-up report to BCT, BCT submits report to Div

CARD 12: Oil Line Sabotage Insurgents attack oil line and disrupt flow
1. Unit conducts immediate actions and sends report to BCT 1a. Immediate actions by unit: (1) SECURE THE SITE!! (2) Contact local power facility supervisor (3) Determine source of failure (4) Report to BCT TOC (5) If oil flow can not be restored immediately, request assistance from higher 2. CMO and CA determine the best method to restore power if the power facility can not repair itself. 2a. SPOT report to BCT TOC: Battle CPT / Battle NCO gathers info: S: Size (1) How large is the physical area effected by the oil loss (2) Size of population effected A: Activity (1) What was the cause of the oil loss? 1a. Immediate actions by unit: (1) Unit secures site. (2) Deploy engineers to conduct assessment on cache -- obtain as much information as possible: (a) inventory of number of weapons / amount of ammo (b) description of items found (digital photos) (c) number of civilians / vehicular traffic in the area (3) Engineers conduct an assessment: (a) Are there weapons / ammo that can be confiscated for police or security force use? (b) If not, then destroy in place (if safe to do so) (c) If not safe, or if cache is too large, coordinate transportation assets to move munitions to a ASP. Unit must secure site until completely destroyed or moved. Unit can use ICDC/IPS to secure site, but must inventory munitions before and after hand over. UNIT REQUESTS EOD SUPPORT FROM BCT ONLY IF TF ENGINEERS CAN NOT IDENTIFY THE MUNITIONS OR THE MUNITIONS ARE TOO COMPLEX. If EOD is necessary, the unit must secure the site until EOD can respond. 1.
