Scenario 6: “Marlene works on an acute psychiatry ward and has been told by both the psychiatrist and the unit manager that a particular patient must be discharged within the hour to free up a bed. She believes that this patient is not ready for discharge, given that he continues to be actively suicidal. The patient is homeless, has no social supports, and no follow-up has been arranged. He has been banned by all local shelters because of his past threatening behavior. He has been on the unit for the past 72 hours, but the psychiatrist will not re-certify him and does not want him as a voluntary patient. The patient wants to continue the work he started with the social worker, Marlene, however. She decides not to challenge the decision to discharge because she is afraid of being fired or at the very least blacklisted by the hospital. Engagement Greet the client Demonstrate effective attending skills Listen carefully Make eye contact (aware of cultural differences) Focus on client thoughts and feelings (use open ended questions) Use silence as necessary Take notes of information you are unlikely to remember Discuss agency services and client expectations Decide if the agency and worker can help Offer agency and worker services to the client Orient the client to the helping process Client needs to know the rules and conditions Negotiate with clients as to frequency of sessions, time and place, and total number of sessions. Complete required paperwork Assessment (Acquire an understanding of a problem or issue, what causes it, and what can be changed to minimize or resolve it) Articulate a clear statement of the need, problem, or situation Recognize the strengths, assets, skills, and abilities of clients Formulate a clear description of the client system Understand the client system’s interactions with other systems Identify any missing information that is important Put all the information together

24 Let’s Practice… In your groups, role-play create a Goal Attainment Scaling Sheet (sample on page 450) with one goal for a mock client. The client’s diagnosis is panic disorder with agoraphobia (panic attacks with fear of leaving home/being in a situation where attacks could happen). Assume for this role play that the client has already expressed her most pressing problem as missing work due to panic attacks and fear of leaving home to attend work. This primary problem is already rated with a weight of 10. Your job is to work with the client to create the levels of success for this problem. See the next slide for a blank GAS form to reference.
