The Examiner Record Booklet is used to record and summarize the examinee’s oral reading performance. Section 1 Identifying Information Section 2 Record of GORT-5 Scores Section 3 Performance Summary Section 4 Descriptive Terms Corresponding to Scaled and Index Scores Examiner’s Manual p. 21

3 Section 1. Identifying Information
Record examinee’s Name Gender Grade School Information Calculate chronological age (age at time of testing) Examiner’s Manual p. 22 Link on class wiki.

4 Section 2. Record of GORT-5 Scores
Transfer each story’s Rate, Accuracy, Fluency, and Comprehension scores from the inside of the booklet to Section 2. Check all Fluency scores to be sure Rate and Accuracy have been summed correctly. For stories not read due to basal 5 pt. credit for each Rate, Accuracy, and Comprehension score 10 pt. Credit for each Fluency score See Sample Record Booklet (slide 5) Examiner’s Manual p. 23

6 Section 3. Performance Summary
Compute the raw total Rate, Accuracy, Fluency, and Comprehension Scores in Section 2. Transcribe to the corresponding blanks in the Raw Total column of Section 3. Convert raw total scores to the following using Appendix A Age equivalents Grade equivalents Examiner’s Manual p. 23

7 Section 3. Performance Summary
Convert raw total scores to the following using Appendix B Percentile ranks Scaled scores Record descriptive terms for the scaled scores using Section 4 of the Examinee Record Booklet.

Sum the Fluency and Comprehension Scores (in ovals). Record the total in the rectangle labeled Sum of Scaled Scores. See Sample Record Booklet (slide 5) Convert this score to the following using Index D An Oral Reading Percentile Rank An Oral Reading Index (ORI) Examiner’s Manual pp

9 Section 4. Descriptive Terms Corresponding to Scaled and Index Scores
Descriptive terms that correspond to scaled scores and the Oral Reading Index (ORI) are found in Section 4. See Sample Record Booklet (slide 5) The terms range from very poor to very superior. Record the appropriate terms under Descriptive Term for scaled scores and the Oral Reading index in Section 4. Examiner’s Manual p. 24
