1 WTU Self-assessment Validation

2 Self-assessment Dashboard
Active – Soldier has open Self-assessment that has not been completed. Click “Active” to go to Soldier Contact Information page Pending Validation – Soldier has completed the Self-assessment and it is ready for validation. Click Soldier name to go to the Validation page Pending PSG/SL Validation – NCM has completed validation, but Self-assessment process is not yet complete (only visible to NCM) Pending NCM Validation – PSG/SL has completed validation, but Self-assessment process is not yet complete (only visible to SL/PSG) A new dashboard has been added to the home page for Squad Leaders and Platoon Sergeants. Nurse Case Managers now have a Home page which includes this new dashboard. The Self-Assessments Dashboard displays all “in-process” Self-Assessments including Active, and Pending Validation along with the Due date. The Due date for Self-assessments is 7 days from the time the self-assessment was generated. The Soldier must complete the SA and both validators must validate by the Due Date. Active Self-Assessments are waiting for the Soldier to complete. Clicking the Active hyperlink will take the user to the Soldier’s contact information page. This link is provided for easy access to follow up with Soldiers. Self Assessments Pending Validation are ready to be validated. Click the Soldier’s name hyperlink to jump directly to the validation. Assessment with a status of “Pending PSG/SL validation” will only appear on the Nurse Case Manager’s dashboard. This indicates the assigned Nurse Case Manager has completed the validation, but the Squad Leader has not. Likewise, the “Pending NCM validation” status will only appear for Squad leaders and Platoon Sergeants. No hyperlinks are available for these because the user has already completed the actions they are responsible for.

The policies behind the validation have not changed. In this overview, notice the Category tabs at the top. Each self-assessment category now has its own tab. The Soldier participation selection drop down has not changed. Soldier Self-assessment data including the color, priority and comments has not changed. Validator information is now located below the Soldier’s input A link to open Action Plans in a popup window is available for this current release And, below the Self-assessment area is the future site for full Action Plan integration. Additionally, the “Action Required” radio button has been removed. Action required is automatically indicated for all Amber and Red ratings.

4 Tabs Nurse Case Manager – Clinical
Squad Leader/Platoon Sergeant – Non-Clinical Review Self Assessment validation is broken down into tabs. One for each category in the Soldier’s Self-assessment. The categories in the clinical and non-clinical sections of the Self-assessment have not changed. The Nurse Case Manager is responsible for the 5 categories in the clinical validation including Activities of Daily Living, Health Care, Medication, Pain, and Behavioral Health. At this point, there have not been updates to the language in the Soldier’s self-assessment ratings. The Squad Leader or Platoon Sergeant are responsible for the 12 Non-clinical categories including Work plan, education, employment, weight control, fitness, well being, social, family, financial, housing, administrative support and transportation. The final tab, REVIEW, contains a summary page with all Self-assessment categories and a link to submit the validation. The review tab may be accessed at any time during the validation. Changing tabs will automatically save information completed on a previous tab. This means that the validator can leave and return to the validation without losing their progress anytime before the Self-assessment closes.

5 Begin Validation Launch the Self-assessment validation

User may also navigate to the Self-assessment from the notifications page. The Self-Assessment will open to the first category tab: Work Plan for the Squad Leader or Activities of Daily Living for the NCM.

6 Soldier Participation
Select the Soldier Participation from the drop down. Face to Face Phone Independent Review Select the Soldier Participation from the drop down box. Face to face or phone participation is the best practice. If it is not possible to discuss the validation with the Soldier, select Independent Review.

7 Review Soldier Input Review the Soldier’s input
Assessment – Green, Amber, Red Priority – Low, Medium, High Soldier Comments – Soldier is required to comment for Amber and Red assessments Review the Soldier’s input. According to the Soldier Leader Guide: Green: Indicates that a Soldier feels that he has everything in place related to the specified field and does not believe he needs additional assistance. Amber: Indicates the Soldier may need assistance or believes that there are obstacles that may limit his ability to get everything in place for that field. Red: Indicates the Soldier is having significant issues and needs assistance. Priority defaults to low for green assessments. For amber and red, the Soldier selects the priority level: Low, Medium, or High. Comments are required from the Soldier if the assessment is not green. This means that Soldiers who are trying to finish quickly may answer green to everything when, in fact, they may need assistance.

8 Validation Actions Validator must check Concur box to concur with Soldier assessment. Validator Comments are required if the Concur box is not checked. Validator Comments are optional if the Concur box is checked. Minimum requirement to complete each tab: Concur box is checked or Validator Comments are entered The Concur action has changed. It is no longer a radio button. The user MUST perform the action of checking the concur check box to concur with the Soldier’s Self-assessment. Not checking the box indicates that the validator does not concur. If the box is not checked, validator comments are required. If the concur box is checked, validator comments are optional. Validator comments are used to identify why the validator does not agree with the Soldier, to clarify the Soldier’s self-assessment, or to document any additional information. They are not a substitute for Action Plans. With this release, the minimum required action for each tab is to Check the concur box or Enter validator comments

9 Action Plans According to policy, Action Plans are required for Amber and Red Self-assessments. Use the Action Plan link under the Validator section to open Action Plans in a new window. Update existing plans or create a new plan as necessary. Future development will bring creation and updating of Action Plans directly into the validation process. For now, continue to document Action Plans through the Action Plans section of the soldier’s case. A quick link to Action Plans is available on the validation page.

Create the Action Plan. CLOSE the Action Plans window to return to the Validation.

11 Tab Completed Select another tab to view another category.
If the minimum requirements were met for the previous tab, a green check will appear. When the first tab is complete, move to another tab. Tabs can be validated in whatever order the user prefers. If the minimum requirements have been met (concur box is checked or comments are added), a green check mark will appear in the completed tab.

12 Review Once all tabs contain the green check, navigate to the review tab. When all tabs contain the green check, the validation can be reviewed and submitted. Select the review tab.

13 Review and Submit Review the validation.
Click Submit to complete the validation. The review tab contains the full self-assessment – both clinical and non-clinical. It can be viewed at any time during the validation process. To complete the validation, navigate to the review tab. Review the validation information. When all tabs have the green check and Soldier Participation is selected, the validation must be submitted. Select the Submit link in the upper right. NOTE: once submit is clicked, the validation is complete and the user is taken to the Home page.

14 Troubleshooting If any steps were missed in the validation, clicking Submit will open a warning pane. Correct the issues identified in the warning and Submit again. If any steps were missed during the validation process, clicking submit will open a warning pane. Correct each of the identified issues, then return to the Review tab and Submit again. When the validation is complete, the user will be returned to the Home page and the dashboard will be updated.
