Interprofessional Collaborative Practice Competencies

Cognition and Mobility Care Management (CMCM) Program
Co-Management Program Screening for cognitive and/or mobility issues of patients age 70 or older CMCM is in select teaching clinics and performs home visits CMCM helps: With co-management of patients with cognitive and/or mobility issues Train primary care clinic staff on screening for dementia and falls Answer questions about memory loss, behavioral issues, falls, caregiver stress, etc. Bridge medical care with community-based resources CMCM Manager: Amy Woodard, BSN, RN, PHN Contact Amy: (951) or
