Bronze: Awareness of quality improvement approach. Board: Able to lead system-wide quality improvement and assurance. Silver: Able to apply quality improvement approach with support. Gold: Able to apply quality improvement approach independently and support others.

8 A standard language for improvement

9 How are we doing? Over 3,000 staff have completed IQT bronze.
10 Silver cohorts held across Wales to date – over 100 improvement projects. Gold network attendance of 80 staff with 50 of those identified as core to Delivery. Board development sessions. Across Wales, 3014 staff have already completed the bronze level which provides an awareness of quality improvement methods and your role in improving care. Some NHS Wales staff have already been using the skills being introduced and patients and staff have already seen the difference improvements introduced by IQT have made. Approximately 100 silver projects are underway across health boards and trusts. Staff who lead teams are focusing on applying the methodology to deliver quality improvements in the workplace. The Gold network is bringing together 80 staff who have a detailed understanding and experience of managing improvement projects, providing skills to enable them to become quality improvement coaches.

10 The Patient Flow Programme
Health boards currently report issues with unscheduled care with breach of 4 hour targets, ‘bed blocking’, above average length of stay and poor patient outcomes. Supporting organisations to address barriers that hinder patient flow through the health and social care system, and ultimately lead to poor patient outcomes and increased mortality rates. It has delivered: Guidance on producing data to analyse flow (the ‘Warwick Charts’). A seminar for Senior Operational Managers with Dr Kate Silvester to understand system flow. Foundation of Improvement Science in Healthcare (FISH) programme delivered for each health system and coaching. The service has requested support through a collaborative approach, with the first learning session on 16 December. A&E breaches & Non elective deaths / discharges by date of admission

11 Client management system
Take stock and respond to service and policy needs. Deploy the resource in the most effective, efficient and cost-effective way to meet local and national needs. Respond in a timely and appropriate way to bespoke requests, family requests and emergencies. Focus on capability, exit strategy and sustainability. Quarterly meetings with the service.

Improving cancer targets and care pathways in Velindre. Supporting the development of the local faculty in AB UHB. Implementing S.T.O.P. in BCU HB to reduce catheterisation rates for orthopaedic patients. Improving surgical pathways and applying human factors in Hywel Dda HB. Supporting an OD exercise to improve the internal organisation of the radiology department in C&V UHB. Developing a Mental Health model aligned to primary care and chronic conditions management in Powys tLHB.
