1 The “H-response” is a prominent photic driving response at flash rates beyond 20 Hz. In a critical review of the literature, the reported sensitivity of the H-response varied from 25% to 100%, and the specificity from 80% to 91% for migraine headaches. Although the relatively high sensitivities and specificities reported suggest that the H-response may be effective in distinguishing migraine patients from controls, and possibly migraineurs from tension headache sufferers, the Quality Standards Subcommittee (QSS) of the American Academy of Neurology concluded that the H-response was not more effective than the neurological history and examination in diagnosing headaches and not recommended in clinical practice (Quality Standard Subcommittee, 1995). However, in the presence of complex or prolonged aura, visual hallucinations, disorders of consciousness, history of recent trauma, and in infants with vomiting and ocular and head deviation, the EEG may be useful for clinical diagnosis and for monitoring therapeutic response.68 Source: Generalized Epilepsy, Atlas of Pediatric EEG Citation: Laoprasert P.
