1. Maineville Primary T-Shirt Order Form
The Maineville PTO is conducting a T-Shirt Design contest!
Each student has a chance to turn in their design. Then, each classroom will
choose their favorite design. Winning designs from each classroom will be posted
and every student will get to vote on their favorite design.
The Maineville PTO will be ordering T-Shirts for the entire student body.
T-shirts will feature the school’s winning design on the front and the student’s
class name and sponsors’ logos on the back. Each classroom will have a
different color shirt. Shirts are $6.00 each.
Please return this form and payment by Friday, September 22nd.
Your payment of $6.00 will help to cover the cost of the t-shirts.
