6 LAUSD/DCFS Data Match The following fields are available on the DataMatch list: Educational Right Holder (HOER) name, and Phone Attorney Name, Phone, Title, Address, CSW SCSW Name, , Phone What about caregiver home address and contact information- is that there?

7 Partnership: FYAP and FamilySource
FYAP and Family Source counselors are to work with students from DCFS DataMatch list and CalPads. FYAP Counselors are assigned open court cases and Family Source are assigned voluntary cases. Transferring cases: On a monthly basis, FamilySource will provide a list of cases that have transferred from Voluntary to Open Court status. FamilySource and FYAP PSA Counselor are to consult and release CAA assessment. If FamilySource has completed the CAA assessment, FYAP is to mark “Yes- transferred-completed” on SOS log caseload tab. If FamilySource has not started a CAA assessment, Family Source is to mark “No”. FYAP Counselors are to mark “No” on SOS log, and complete the CAA. If FamilySource has started the CAA and not finished the CAA, FamilySource is to mark “In-Progress”. FYAP Counselors are to mark “Transferred-In Progress”, and complete the CAA.

Student Community Stakeholder LAUSD/LEA Stakeholder Contact is required if school or residential placement is at risk of being changed. Include one type of contact: Individual student contact Classroom observation Group observation Playground observation Include one contact from the following: Caregiver – Biological Parent, Foster Parent, Group Home Staff Educational Rights Holder – CASA, Surrogate CSW (due diligence) Form of contact: In-person Phone call Individualized s Individualized Letters Include a contact from one of the following: Teacher(s) DIS provider if 1989 Transition Service Facilitator (TSF) if 1017 School administrator Form of contact: In-person Phone call Individualized s Individualized Letters

Student Community Stakeholder LAUSD/LEA Stakeholder Contact is required if at-risk indicators/noted concerns have been identified or if requested by the caregiver or any stakeholder. Exception: The student has been withdrawn/checked out of school and the FYAP Counselor is verifying enrollment and assisting with records transfer, as needed. Include one type of contact: Individual Student Contact Classroom Observation Group Observation Playground Observation Include one contact from the following: Caregiver – Biological Parent, Foster Parent, Group Home Staff Educational Rights Holder – CASA, Surrogate CSW (due diligence) Form of contact: In-person Phone call Individualized s Individualized Letters Include a contact from one of the following: Teachers School administrator Support personnel: PSW, PSA, Bridge Coordinator, School Counselor, 1-on-1 Aide, etc… Form of contact: In-person Phone call Individualized s Individualized Letters

Counselor Spot Light Acknowledged by Arleta High School for outstanding work with foster youth. Foster Youth on track to pass all A-G courses with a “C” or better increased from 19% to 36% Percentage of Foster Youth with an annually updated Comprehensive Academic Assessment for school year was 92% for Arleta High School compared to LAUSD Average of 68% FYAP Counselor: Jay Longo When was the baseline of 19% taken?

Question: How can I view School Report Card Answer: Log onto Question: When will School Report Card be available? Answer: Available to principals Monday, October 26 Available on LAUSD School Report Card website Monday, November 30 Mailed to parents after Monday, January 18 Why are we reviewing this again now- what is the connection?
