2 What is Trigger Tool? New learning package that replaces the Older Person’s Wellbeing Trigger Tool training Now focuses on all adult groups Focus on prevention messages and supporting independence Promotes Connect to Support Stronger focus on carers

3 Learning objectives: Identify some of the risks that adults may have in their homes Feel more confident in talking to the person about these risks Know how to work with someone in a strengths based way to ensure that they are able to participate fully in their community Know where you can access support for someone in your local area

4 The Care Act 2014 Duty on Local Authorities to promote an individual’s wellbeing LA must provide services to prevent, delay or reduce the need for care and support LA must work closely with local health authorities LA must provide information and advice LA must provide a variety of care

5 The strengths based approach
What people can do for themselves Instead of what do they need, what do they have that can be built on Identifying lost skills, past experiences Once strengths are identified, these become an asset which they can offer to other people Everyone has something to offer and has a strength of some description Support from local networks, including family and neighbours

6 Positive risk taking Some of the Trigger Tool training focuses on ‘risk’ Risks aren’t always negative Positive risk taking involves weighing up the benefit of doing something that may not work against the potential benefit that can be gained If we don’t take risks we miss opportunities

7 Section One – Safe at Home
Focuses on: Scams and doorstep crime Fuel poverty Hate and mate crime Budgeting and finance – benefits calculator Falls prevention Communication and checking understanding

8 Section Two – Health and Wellbeing
What is the difference between Health and Wellbeing? Prevention public health messages Nutrition including malnutrition and hydration Exercise Mental health Dementia Think carer

9 Section Three – Supporting Independence
Focusses on: Loneliness and social isolation How these link to wellbeing and health problems How to spot the signs of isolation Showcases some of the support available in Hampshire Promotes telecare

10 Links and certificates
Connect to Support – care and support information, directory of local services, information on care homes Links to other local and national providers of information and advice Printable version of Hampshire services Certificate Community Trigger Tool training
