Why is it important to report accident and potential hazards to the appropriate person? From ELLIE C, ELLIE M & ZAYNAH A.

2 Accident book? An accident book is a book which records all accidents which take place within a work environment, this is important for the sake of the business’s safety for example a employee could slip on a wet floor without there being a caution sign and try to sue the company for not taking safety precautions. What types of things should be reported? Tripping over an open desk or file drawer, electrical cords or wires, loose carpeting, or objects in hallways/walkways. Bending or reaching for something while seated in an unstable chair. Using a chair in place of a ladder. Slipping on wet floors. Inadequate lighting.

3 What would happen in the event of a disease?
Mention how RIDDOR covers this? Deaths and injuries If someone has died or has been injured because of a work-related accident this may have to be reported. Not all accidents need to be reported, other than for certain gas incidents, a RIDDOR report is required only when: the accident is work related it results in an injury of a type which is reportable

4 What forms would you need to fill out if you were to have a near miss?
The form you would have to fill out if you were to have a near miss would be the near miss report form. What’s a NEAR MISS? A near miss is an event that could have caused a serious injury or illness, but didn’t. For example: Someone spills coffee or water on the floor and does not clean it up. Someone slips but does not fall. A forklift operator takes a turn too quickly and drops the load which almost hits a nearby worker. Would you also phone the relevant people You would phone your manager to inform them, you would have to tell them to prevent it occurring to another colleague in order for others within the environment to maintain safety. When should you have this form filled in by? This should be filled in straight away in order for the time of the accident to be acknowledged in case of further investigation.

5 Whose responsibility is it to uphold elements of the COSHH?
It is the employers responsibility to control substances that are hazardous to health you can prevent or reduce workers exposure to hazardous substances by Finding out what the health hazards are Deciding how to prevent harm to health planning for emergencies Providing monitoring and health surveillance in appropriate cases.

6 How is COSHH related to HASWA?
The main purpose of COSHH is for employers to control dangerous substances therefore this relates to HASWA as HASWA is also the health and safety at work act 1974 and covers occupational health and safety in the UK.

7 Why is it important to wear personal protective clothing to follow these acts when dealing with hazardous substances? The importance of wearing special clothing when dealing with strong or dangerous substances is key because there could be certain chemicals which could harm you such as burn your skin, or affect your eyes if goggles are not worn. Your clothing could also be ruined if the correct clothing is not worn. Chemicals could possibly be spilt and cause major consequences which could be dangerous to ones health.
