6 Assessment It’s a on-going process. Formal and informal approaches.
What Is It? It’s a on-going process. Formal and informal approaches. Tools include applications, testing and interviews. Assessments should drive the services to be provided. Assessments are not a tool for screening out applicants.

7 Assessment For the client: Provides evidence of where they are now.
What Is It? For the client: Provides evidence of where they are now. Also, its motivational in terms of identifying strengths, barriers and opportunities.

8 Assessment For the case manager:
What Is It? For the case manager: Describes the gap between where the client is now in relation to the goal of improved self-sufficiency. It identifies the client’s strengths, motivators, barriers. Determines eligibility. Determines suitability: Will client benefit from case management services?

When Does It Begin? Assessment begins at first contact. This is referred to as initial assessment.

31 Assessment Tools Tests can reveal skill and ability levels of individuals, indicating training needs. Assessment tools can also be used to predict an individual’s success in training. Skill or ability tests can also be used to assess an individual’s progress in training, or to evaluate the effectiveness of a training program.
