2 Lockout-Tagout Procedure
CSP shall establish a program consisting of: Energy control procedures Employee training Periodic inspections This procedure for the control of hazardous energy shall comply with the OSHA 29 CFR and shall clearly outline: The scope Purpose Authorization Rules Techniques

3 Lockout -Tagout Procedure
The procedure shall ensure that before any employee performs service or maintenance on equipment, the equipment be isolated when an injury could occur do to the following: Unexpected energizing Unexpected startup Release of stored energy (pneumatic, mechanical, electric, chemical)

4 Lockout -Tagout Procedure
The procedure shall also define the means to enforce compliance as follows: A specific statement of the intended use of the procedure Specific procedures for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy Specific procedural steps for the placement, removal and transfer of lockout devices, and tagout devices and the responsibility for them Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures

5 Lockout -Tagout Devices
Lockout and tagout devices shall be capable of withstanding the environment to which they are exposed Tags shall not deteriorate when used in corrosive environments Lockout and tagout devises shall be standardized within CSP Lock out devices shall be substantial enough to prevent removal without the use of unusual techniques or excessive force

6 Lockout -Tagout Devices
Tagout devices, including their means of attachment, shall be substantial enough to prevent inadvertent or accidental removal. Tagout device attachment means shall be of a non- reusable type, attachable by hand, self-locking, and non-releasable with a minimum unlocking strength of no less than 50 pounds and having the general design and basic characteristics of being at least equivalent to a one-piece, all environment-tolerant nylon cable tie.

7 Lockout - Tag Out Devices
Lockout devices and tagout devices shall indicate the identity of the employee applying the device(s) Tagout devices shall warn against hazardous conditions if the machine or equipment is energized and shall include a legend such as the following: Do Not Start. Do Not Open. Do Not Close. Do Not Energize. Do Not Operate.

Authorized employee must have his/her own device Devices must be used to control energy only Devices could be: Locks Tags Chains Wedges Key blocks Adapter pins Self-locking fasteners

Cable lockouts Circuit breaker lockouts Group lockout boxes Locks Lockout centers and stations Lockout hasps Valve lockout devices

10 Lockout - Tagout Devices
Group Lockout: Each authorized employee must affix a personal lockout or tag-out device Each authorized employee must remove the device when work stops

11 Periodic Inspection The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of the standard are being followed The periodic inspection shall be performed by an authorized employee other than the one utilizing the energy control procedure being inspected

12 Periodic Inspection The periodic inspection shall be conducted to correct any deviations or inadequacies identified. The employer shall certify that the periodic inspections have been performed. The certification shall identify: The machine or equipment on which the energy control procedure was being utilized The date of inspection The employees included in the inspection The person performing the inspection

13 Training and Communication
The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. Each affected employee shall be instructed in the purpose and use of the energy control procedure

14 Definitions: Lockout Lockout: Process of preventing the flow of energy
Consists of installing a lock, block, or chain Requires an Authorized Employee to apply/remove lock

15 Definitions: Tag Out Tag Out: Process of placing a warning tag on the energy source The tag acts as a warning The tag must clearly state “Do not operate” The tag can be used when the equipment won’t accept lock The tag must be applied/removed by an Authorized Employee

16 Definitions: Authorized and Affected employee
Authorized employee: A person who locks out or tags out machines before the servicing or maintenance of equipment can proceed. Affected employee: An employee who will do work on the equipment An employee who works in the area Cannot help authorized employees service machinery

17 Definitions: Energy Source
Energy sources can take several forms: Electrical Mechanical Pneumatic Hydraulic Chemical Thermal Water under pressure Gravity Potential energy

18 Definitions The following is a partial list of activities that require LOTO: Constructing Installing Setting up Adjusting Inspecting Modifying Repairing Cleaning equipment

19 Definitions Centrifugal Pumps Agitators Hoists Dry Air Compressors
The following is a partial list of equipment subject to Lock Out / Tag Out in CSP Centrifugal Pumps Agitators Dry Air Compressors Wet Air Compressor Water Heater Local Control Panel Hoists Rectifier Roll up Door Ion Exchange Columns Metering Pumps Filters Any other equipment for which maintenance activities represent a risk to the maintenance operator

20 LOTO: Steps The following steps apply to the lockout/Tag Out procedure
Prepare for shutdown Shutdown Isolate equipment from energy source Apply Lock Out or Tag Out device Release of stored energy Verify isolation

21 LOTO: Steps 1. Prepare for Shutdown
Know the type and magnitude of energy source Know the hazards of the energy source Know the methods or means to control it 2. Shutdown Turn off equipment using normal controls Use orderly shutdown

22 LOTO: Steps 3. Isolate Equipment Isolate equipment from energy sources
Secure lock to energy isolating device

23 LOTO: Steps 4. Notify affected employees Attach lockout/tagout devices
Only authorized employee can attach or remove lockout and tag-out devices Devices must be durable Devices must identify person who applied them

24 LOTO: Steps 5. Release Stored Energy Relieve stored energy
Verify machine isolation from energy Relieve pressure in compressed air line Add interference elements if applicable 6. Verify Isolation Verify that the machine is de-energized Verify that the machine is isolated Verify that stored energy has been released

25 Device Removal If devices must remain in place after shift is over:
Lockout/Tag Out protection must be continuous Lockout/Tag Out protection must have an orderly transfer between employees

26 Device Removal The lockout or tagout device must be removed by the authorized employee who applied it. The lock-out device may be removed under direction of employer if authorized employee unavailable.

27 Device Removal Steps employers must take before removing a lock or tag: Verify that authorized employee who applied lock is not at the facility Take all reasonable efforts to contact the authorized employee

28 Device Removal If crucial to remove, and employee cannot be reached, call the Plant Manager or the Maintenance Operator. Inspect the work area Ensure all employees are safe Notify all affected employees
