3 Once the group has completed quoting and underwriting, and the employer has reviewed prescreen rates, the new business process begins.

4 New Business Documents
During the underwriting stage, the employer and employee disclosures and/or forms should have been completed and submitted. Additional documents required for new business processing include: Group Setup Form ACH Processing Form Administrative Services Agreement Writing Agent Transmittal The group’s most recent quarterly wage and tax statement

5 New Business Documents: Group Setup Form
Requests employer information, including: Company type Insurance contact information COBRA and retiree information Authorizations for key personnel

6 New Business Documents: ACH Payment Processing Form
Employers electing SafeGuard coverage must make monthly payments via automatic account withdrawals This form collects all account information necessary to set up the automatic deductions, as well as required authorization

7 New Business Documents: Quarterly Wage/Tax Report
Employers must submit the most recent quarterly wage/tax report for verification of participation and validation of eligibility

8 New Business Documents: Administrative Services Agreement
This document establishes the administrative services contract which is to exist between USHL and the employer All areas of services to be provided are identified and defined

9 New Business Documents: Writing Agent Transmittal
Information pertaining to the writing agent for the group, necessary for commission payments

10 New business documents are processed by underwriting and final rates are issued– these will match prescreen rates if there were no significant changes in the group census from the time of quoting.

11 Offer Delivery Two documents constitute SafeGuard offer delivery:
Offer of Excess Loss/Policy Schedule Excess Loss Insurance Application

12 Offer Delivery In order to accept the terms and conditions of the SafeGuard offer, the employer must: Sign and return the excess loss insurance application (electronic delivery is acceptable) Mail a check for the first month’s payment to US Health and Life Insurance Co. Underwriting Department 255 S Old Woodward Ave, Suite 300 Birmingham, MI

13 Enrollment Once offer acceptance is received, along with the first month’s payment, enrollment begins Within 48 to 72 business hours, participants are enrolled with USHL and network(s) Subsequently: A welcome letter and administrative kit goes out electronically to the group and agent The excess loss insurance policy is mailed to the group Group contact receives MPD and SPDs Members receive ID cards
