2 Introduction to the Phone Enrollment Experience
Surebridge is proud to announce a new sales tool which builds on our previous successes. The tool is faster, more capable, and provides better reporting than the previous tool. The following slides go over the major changes, and then a brief walkthrough of the in-person Enrollment Process. 3 Introduction to the Voice Signature Process Improvements
Application We have moved the Enrollment Selection Method to the beginning of the apply process. This will reduce re-reading information twice via the phone. When applying for Supplemental and Life we have streamlined the process to be one flow. All supplemental then life underwriting questions will be asked; then all billing information, etc. We are now using DocuSign as our means of obtaining acknowledgments and signatures. This provides added security for your applicants and more ease in obtaining signatures. It is imperative that the primary applicants mobile number is entered. A text will be generated to them for use within DocuSign. New subway will help you know where you are in the process. See below: ©2016 SureBridge | Proprietary and Confidential.

5 Voice Signature Process Select Signature Type
The phone process has two options to capture the customer signature. Obtain the customer signature by directing the customer via to an online site containing all application documents and disclosures. Secure voice recording is not required for this method. Voice: Obtain the customer signature via voice recording. This method will require you to record the entire conversation on our secure voice recording line indicated below AND you must read aloud and verbatim all required documents and disclosures as indicated. Dial and enter the six-digit Call ID code provided, then enter your agent id and conference in the applicant. We will be completing your application for supplemental insurance via the phone and recording all information including your signature. From this point on, this call is being recorded for quality and compliance purposes. Are you ready to proceed? Phone / Voice Voice This is <YOUR NAME> with the <GENERAL AGENCY NAME> and I am speaking with <NAME OF APPLICANT>. You acknowledge that you are the proposed Applicant, owner or Payor of the insurance contract. You understand you are applying for Chesapeake SureBridge supplemental and/or life insurance and Voice You understand these policies are not health insurance , are optional for an additional premium and are not required in order to purchase health insurance, and You understand this acknowledgement does not guarantee that policies will be issued, as such policies are subject to eligibility and underwriting requirements of Chesapeake and coverage is not effective unless and until you are informed in writing that your application has been processed and approved, and Premium rates may vary based on factors such as benefit level(s) selected, payment mode selected, family composition and policy issue date. Actual premium rates are not final until coverage is issued. 12 Application Eligibility and Underwriting Questions - Life

15 Voice Signature Process Read Required Forms for Voice Signature
Verification of voice signature is completed with by reading these forms verbatim. Click on view document, read the entire document, then click on the acknowledgement. When all documents are complete, save and continue is enabled to proceed. At this point, the voice recording must end and the agent collects the payment information.
