1 “PRACTICE BASED ASSESSMENTS” An update for 2017/18
Dr Sabir Zaman Clinical Teaching Fellow CBME Year 5 Academic Lead We all stand to gain from improved feedback and evaluation. For tutors, this would help you to understand why something did –or didn’t work- and enable you to develop as educators. For students, engaging with the feedback process in a more thoughtful and reflective way will help them to develop skills that they will put to continuous use as they embark on their medical careers. And faculty also stands to gain, as this could highlight areas for potential curriculum development.

3 Year 5 Community Care Placement
6 week block Supervised Clinics Independent Consulting in a safe setting Some Professional Shadowing Tutorial time Primary Care Experience MDT Continuity of Care Part of the Practice Team

4 Aims of the Placement To help all students to become better doctors, irrespective of their current career preference or eventual career choice. To provide an introduction to General Practice and Primary Care in all its facets and to help the student experience independent practice whilst in a safe learning environment. Specific Objectives outlined in Student Handbook on QMPlus

Professionalism – punctuality, reliability, team-player Practice Based Project Patient Case and Reflection on Professional Practice Principles Letter Writing Exercise Two CEXs – not to be uploaded

6 Other Requirements of the Placement
Attendance at Introductory and Closing Days Attendance at Hospice Morning – St Joseph’s Hospice A 4 hour ‘Out Of Hours’ OR ‘Walk-In Centre’ session – advised to book early

7 Other Assessments During Year 5
Prescribing Safety Assessment (PSA) – National Exam Situational Judgement Test (SJT) – National Exam, Dec 2017 or Jan 2018 Finals – MBBS Exam, March 2018

In small groups spend a few minutes discussing your experience of Practice Based Assessment during the Year 5 Community Care Placement, and any challenges you may have faced

11 Patient Case and Reflection
A write up and a discussion between the Student and Tutor Both aspects contribute towards the task Tutors can support students in selecting an appropriate case Show reflection on two of the “GMC Duties of a Good Doctor” criteria Include at least one of either ‘Relationship with Colleagues’, or ‘Relationship with Patients’ 1000 words including a short (300 word) case summary

12 Practice Based Project
All students expected to produce an individual piece of work Tutor to agree a task that is related to the Practice Team Task should aim to help improve patient care in some way Students should present their findings at the end of the Placement to their Tutor and the Practice Team Examples have included Audits, Patient Information Leaflets, Posters, Surveys, Care Plans Can be innovative

13 Practice Based Project
Development of new GP Prize to be presented at Graduation Initiative of new Professor of Primary Care Support from RCGP Faculty Final details currently being developed, further information to follow As all students must submit this piece of work, all are eligible for the prize Key to promoting General Practice to our students and faculty

14 Complex Care Plan Focus on Chronic and Complex Care in General Practice An area particular to this placement Aim is to assess skills in gathering and communicating information to patients Opportunity for students to assess a case in detail – patient should have chronic and complex needs, more than one chronic condition, and multiple medications Students should spend time interviewing and examining a patient, and review the medical record Care Plan should be written for the patient – maximum two sides of A4 paper

15 Quality Improvement in Action
Previously Ethics in Action worksheet Was discussed at ‘Ethics in Action’ workshop at Closing Day Students invariably did not complete this worksheet Change in focus to ‘Quality Improvement in Action Students should consider areas where they have seen improvement – following a complaint, an audit, a meeting, a clinical encounter, an ethical dilemma All students must complete the worksheet and show their tutors, this will be part of the final assessment form

16 Clinical Evaluation Exercise
Clinical Evaluation Exercise (CEX) Observed Student Consultations Two to be done – Week 1 and Week 6 Objective is to see an improvement between these two evaluations Do not need to be submitted to the Medical School Copies to be given to student for their records

17 Summary Changes have been made to the assessments for 2017/18
More consideration of Complex and Chronic Care More consideration of Quality Improvement in Action Proposal for a new GP Prize Changes to the curriculum in the near future will result in another review
