2 Why Integrate…? Often described as to address the three main challenges facing health and social care An aging population More people living with multiple, complex, long-term conditions; and Ever reducing public funding. But also:- The old way of working did not deliver services that naturally contributed to people being able to live a ‘good life’ and didn’t always work for people

3 How to Address the Challenges…?
Create a new system that: - Is joined-up from the perspective of people who use services Makes it easier for money to flow between the different parts of the system Offers services, which are built around the needs and aspirations of people who use and / or need services Helps people stay well and get support earlier when they need it Designs & delivers services that positively impact on people’s lives and the communities they live in Implicit in this, and explicit in the legislation, is the need for the system to coproduces its priorities with people and communities

4 1. People are able to look after and improve their own health and wellbeing and live in good health for longer. 2. People, including those with disabilities or long term conditions or who are frail are able to live, as far as reasonably practicable, independently and at home or in a homely setting in their community 3. People who use health and social care services have positive experiences of those services, and have their dignity respected 4. Health and social care services are centred on helping to maintain or improve the quality of life of people who use those services 5. Health and social care services contribute to reducing health inequalities. 6. People who provide unpaid care are supported to look after their own health and wellbeing, including to reduce any negative impact of their caring role on their own health and wellbeing 7. People using health and social care services are safe from harm 8. People who work in health and social care services feel engaged with the work they do and are supported to continuously improve the information, support, care and treatment they provide. 9. Resources are used effectively and efficiently in the provision of health and social care services.

5 What MUST Be Integrated…?
All adult services previously managed by the Local Authority Social Work Department or CHCP (NOT criminal justice social work) including assessment, services and support for older people, adults with disabilities and mental health issues, adults in recovery and unpaid carers All adult health services previously managed by the CHP or CHCP including the work of health visitors, district nurses and community based allied health professionals (e.g. physios, OTs, dieticians and speech & language therapists); community based mental health services, palliative care, long term condition support; and the relationship with primary care contractors (including GPs, opticians, pharmacists and dentists) The full list includes some services that were previously provided in part by one or the other, and were integrated under all or some CHCP such as aids and adaptations, including telehealth and telecare; health improvement activity; and community hospitals

6 What MUST Be Integrated…?
Plus: Hospital services that most commonly associated with the emergency care pathway (i.e. areas where unplanned admissions dominate) A & E General medicine Geriatric medicine Rehabilitation medicine Respiratory medicine Palliative care services Learning disability psychiatry Addiction services Mental health services (except secure forensic mental health services) The inclusion of the planning, delivery and financing of these services within the integrated authorities portfolios means that the role of the new integrated authorities goes far beyond even the furthest reaching CHCP role. Integrated authorities can also choose to include other services e.g. children’s services, criminal justice

Integration Joint Board / Joint Monitoring Committee Strategic Planning Group (sometimes has subgroups for people who present with specific needs e.g. physical disability; learning disability,…) Locality Planning Locality Planning Group A Group B At least 2 but can be as many as 9 (these groups have a focus on specific geographic communities)

10 Doing Things Differently…
Strategic Commissioning All the activities involved in Assessing and forecasting needs, Linking investment to agreed desired outcomes, Considering options, Planning the nature, range and quality of future services, and Working in partnership to put these in place. It is a co-produced approach and sits at the very heart of how change will be enabled

11 Strategic Planning Group
The mechanism through which strategic commissioning for the area is progressed. Responsible for supporting the Partnership to develop the draft strategic commissioning plan and then to implement the final plan and monitor its progress. Its focus is not how things are done now but how things can be done in the future. Membership: Defined in legislation; includes local professionals; people who use health and social care services; unpaid carers; non-commercial providers of health care; non-commercial providers of social care; non-commercial providers of social housing; and third sector bodies carrying out activities related to health or social care. Stipulates that participants must live or support people who live in the area

12 Locality Planning Group
The mechanism through which strategic commissioning for a locality is progressed. Responsible for planning for the local area Their thinking should inform the Strategic Plan but their priorities and how these are delivered also needs to be aligned with the Strategic Plan once it is finalised (what each locality place emphasis on and give highest priority to is likely be different). Membership: not defined in legislation but the guidance defines the same stakeholder groups as strategic planning group and stipulates that participants must live or support people who live in the locality
