Signs and symptoms, Assess history of present illness. Allergic Reaction History of allergies? What were you exposed to? How were you exposed? Progression? Interventions? Respiratory OPQRST? Interventions? Cardiac OPQRST? Interventions? Altered Mental Status Description of the episode. Onset? Duration? Associated symptoms? Evidence of trauma? Interventions? Seizures, Fever?

Environmental emergency Source? Environment? Duration? Loss of consciousness General or local effects? Obstetrics Are you pregnant? How many weeks pregnant? Pain or contractions? Bleeding or discharge? Feel the need to push? Last menstrual period? Behavioral How do you feel? Suicidal tendencies? Patient threat to self or others? Medical problem? Interventions?

After signs and symptoms, what other patient history should be obtained? Any known Allergies? Medications? Past pertinent history Last oral intake Events leading to present illness – Rule out trauma After taking history: Performs focused physical examination – Assesses affected body part or system. If indicated completes rapid assessment.

8 Assessment cont… (4 points)
Following physical exam, what should be done? Vitals – Baseline vitals Interventions – Obtains medical direction or verbalizes standing orders for medication interventions and verbalizes proper additional intervention and treatment. Transport – reevaluates the transport decision Verbalizes the consideration for completing a detailed physical examination.

9 Ongoing assessment – Reassessment (3 points)
As an ongoing assessment you should re-check what? Repeats initial assessment Repeats vital signs Repeats focused assessment – regarding patient complaint or injuries
