4. Infant Safety
Align messaging and efforts about safe sleep, drowning, coping with crying, shaken baby, hot car – coordinate with tb5.
Facilitate CMS referral through WIS/NOS at time of birth if possible.
Coordinate for child care priority – policy and referral processes.
Ongoing patient education.

5. Coordinated Intake and Triage
Case Management page in local tab under development.
Increase use of referral form.
Re-openings need to be staffed/considered from process perspective.
Capacity in care coordination is a constant concern.
Access to prenatal services difficult for women on MAT.

6. Policy Bill drafting to be done by 1-27-17 for this session
Rule for ICC being presented to DCF/DOH
Policy Academy team convenes to to review state policies that can improve prenatal access, child safety, and prevention efforts.
Child care provision, housing, RPICC, CMS, will be included in Policy Academy agenda.

7. Interconception Care
Coordinate with hospital for birth control before discharge
Work on policy for tubal at time of delivery (reimbursement rates, etc.)
Women’s Questionnaire and info about family planning at increased points of entry (child care, Family Place, pediatric facilities, WIC, etc.)
Family Planning Waiver facilitation and coordination with health department.

8. After Care
Coordinate for medications after discharge from residential services.
Housing linkage – centralized intake with homeless services and HMIS.
