2 What is GEOS? The Georgia EPD Online System (GEOS) is a portal for online electronic permit applications including: Brownfield Construction Storm Water Hazardous Substance Fee Hazardous Waste Fee Lead-Based Paint & Asbestos NPDES Municipal Wastewater Scrap Tire Storm Water Industrial Title V Trust Fund Invoice Payment Only Underground Storage Tank Waste Reduction Voluntary Remediation Program New applications will be added to GEOS over time.

5 Account Groups & Privileges
User Type Purpose Account Privileges Responsible Official (RO) Only the RO account can certify and submit applications in GEOS. An RO can only maintain one User Account; this account inherently has Preparer privileges. This one account may be assigned RO privileges for certain facilities and Preparer privileges for others. Generally, for an RO to submit an application, they must first be approved by the EPD as the RO for that facility. An RO can request additional Account Types through the GEOS system Certify and submit a data entry form in GEOS Associate a consultant to their account to prepare applications View and submit Online Payment Track account transactions View and prepare an electronic data entry form in GEOS View submitted data in GEOS Keep track of the status of submitted records Keep track of issuances

6 Identity Proofing In order to be approved as an Responsible Official for a facility you may first have to have your identity proofed The type of Identity Proofing that you must use is dependent on the types of applications you are requesting Identity Proofing is done by e-verify or ESA (Subscriber Agreement) For UST it is recommended that you use the e-verify method

7 Account Creation Navigate to Click the “Create a New Account” link on the login page.

8 Account Creation Enter in your personal identification and contact information. Any field that contains a * is required and must be filled to continue. By placing the mouse over the , a help box will display to provide additional information

9 Account Creation Select the account type reflecting the Application(s)
for which you are responsible. Click ‘Associate New’ to choose the name of your associated master facility from the provided list.

10 Account Creation Some thoughts on creating an RO account
You have to select one or more “Account Types” You have the option to associate one or more facilities with your account. You may choose to do this at a later time. Depending on the “Account Types” you select you will be required to perform an identity proofing for each facility.

11 Account Creation RO Facility Association
After clicking “Associate New” use the Search Master Facility tool to locate your facility Leaving the search field blank and clicking search will return all facilities in the system. Check the facility(s) and application(s) for which you are responsible

12 Account Creation Next you will need to provide answers to a few security questions. These questions will be used later to retrieve a lost password and used to submit applications. Answers are case sensitive!!

13 Account Creation To finish creating your account a CAPTCHA needs to be verified. Once verified, you will need to go through identity proofing. Two methods E-Verify ESA (Subscriber agreement)

14 E-Verify Identity Verification
The E-Verify option is recommended for UST submittals. The E-Verify process completes the verification online If successful, you will be informed promptly and can start using your account to certify/submit documents to the Agency.

15 E-Verify Identity Verification
E-Verify Option You will be asked to provide your home address birth date last 4 digits of Social Security Number The System will then attempt to verify your identity Due to security reasons, the System will only allow you two attempts.

16 ESA Option Identity Verification
You will be asked to print and sign an Electronic Signature Agreement and mail it to the Agency at the address shown on the ESA. The Agency will verify your identity and make a decision on your RO request. You will receive an notification after an Agency’s decision is made. The ESA will take some time because it involves mail delivery, paper handling, human checks, and data entries.

17 ESA Option Identity Verification
Click the “Print Subscriber Agreement” link, GEOS will auto-fill the subscriber agreement form.

18 Account Creation You will receive an notification of the account creation with your login name and randomly generated password. You can use this information to login to the GEOS Public Portal.

19 Password and PIN Once you log back in you will be required to create a password and a PIN number and confirm each of these. Password must have at least one uppercase letter, one lowercase letter and one digit number and be a minimum of 8 characters long. Your PIN number must be at least 4 digits.

20 Submitting an Application
Select a submittal type Select a facility Enter necessary information on the application form Insert attachment(s) Data validation and completeness check Pay Submittal Fee (If needed) Certification and submission Acknowledgement receipt and confirmation

21 Annual Tank Registration
Select the “Start New Application” in the upper left hand side.

22 Annual Tank Registration
Select the “Start” button on the “UST – Storage Tank Registration Form” application.

Select the “Next” button to start the application.

24 Annual Tank Registration
You will now fill out the form. There are eight (8) parts that need to be filled out.

In Part 1, if you answer a question and it indicates that you are in violation of the Georgia Rules for Underground Storage Tank Management you will need to contact EPD to resolve prior to continuing.

26 Annual Tank Registration
Part 2 requires that you have associated your account with and been approved for your facility. You will select your facility from the drop down in the “Facility/Property” list.

27 Annual Tank Registration
In Part 3, you will start by clicking the “Add New Record” button and filling out the embedded form. This information can be found on your operator A certificate. Once you are finished filling out the information click the green check mark to save the data.

28 Annual Tank Registration
Part 4 can be filled out like Part 3. If the information is the same as above, you can simply check the “Same as above” box. This information can be found on your operator B certificate.

29 Annual Tank Registration
In Part 5 you will want to start by clicking on the “Search” button. Here as before when searching you will want to put in only small pieces of information as it will need to match exactly when searching. You want to make sure you select the item with a location identifier filled out and then click “Add to form”.

In Part 6, you will fill out your mechanism to pay for an environmental cleanup should a release occur.

31 Annual Tank Registration
In Part 7, the jobber is who delivers your fuel. In this part, you fill it out by clicking “Add New Record”, filling out the embedded form and clicking the green check mark.

32 Annual Tank Registration
In Part 8, you must review all of the units by clicking the pencil and paper icon.

33 Annual Tank Registration
You will need to set the “Tank Status” or “Piping Status” from the drop down and then click the “Save Unit” button at the bottom of the form. You will need to repeat this process for each of the units in the list.

34 Annual Tank Registration
If you have filled out all of the information that is required, your application should pass validation and look like the image below. If you have errors instead of green checks you will have a red x. You will need to go back and correct these errors prior to proceeding. Click on the “Next” button.

35 Annual Tank Registration
You will need to check the box certifying the submission. You will need to answer the security question and provide your PIN. You will then click the “Submit” button at the bottom of the page to submit the application to EPD.

36 Annual Tank Registration
Once it has been submitted, it will take you to a confirmation screen. Save this for your records. You will need the Submittal ID if you contact EPD with questions.

37 Annual Tank Registration
Once you have gone to the submitted list, you can check the status of your tank registration.

38 Annual Tank Registration
Once the status has changed to “Permit Issued” click on the view button to print your certificate.

39 Annual Tank Registration
Click on the small pdf icon above the Action button to print your certificate.
