Please log on to the CastleBranch website: Use the Information Sheet provided by the School of Nursing (SON) to register for your package. 5 Entrance Requirements
Drug Screening Follow instructions on Castlebranch for submitting a urine sample for drug testing. Drug screening is done at the beginning of the program, you may need to update screening per hospital site requirements – clinical instructors will advise at orientation. Need to notify Dominique Teaford immediately, if there are any changes to your drug screen throughout the program. Background Checks Follow instructions on Castlebranch for submitting a background check. May need to update your background clearance per hospital site requirements during the program. Notify the Director of the Nursing Program Dr. Colleen O’Leary Kelley if you have concerns. Need to notify Dominique Teaford immediately if there are any changes to your background check, drivers license status, etc. Compliance Compliance

6 Castlebranch Requirement
The Affordable Care Act mandates that all citizens have qualified medical insurance. Go to Covered California website for details if you do not have insurance Provide a copy of your Health Insurance card Your name must be on the card – or support documentation are needed. Upload new cards when changes in coverage occur. Insurance must include 24/7 hospital stay and emergency room coverage.

7 Health Statement Download the form from Castlebranch or the SON website. The form needs to be signed by your primary care provider. This form is a statement of your health status and clears you for participation in clinical. Changes to health status include: Injury Pregnancy Mental Health issues Surgery Vision and hearing changes If you have a change in your health status, please notify the Undergraduate Coordinator Dr. Malloy asap. When you are ready to return to course work, you will need to provide a new health statement clearing you for clinical signed by the provider caring for your change in status. ( EX: if you become pregnant and are returning to classes, your OB needs to sign off your health statement)

8 Cardio-Pulmonary Resuscitation (CPR)
Castlebranch Requirements: Basic Life Support for Healthcare Professionals Infant, Child & Adult and AED training (BLS) No online classes are accepted currently. CPR is required Yearly All nursing students (including advanced placement students See following slide) need to have yearly renewal of CPR training. Obtain Certification Between Dec 10 to Jan 10 or in June or July CPR can not expire during the semester

9 Advanced Placement RN to BSN Students Only
If you have a current ACLS and PALS Certification that does not expire during the academic year – you may use those certification instead of Basic Life Support (BLS) But you need to have both. Castlebranch Requirements: You must have both They can not expire during the semester You have to renew yearly while in nursing school Hospital Requirements

10 CPR You will receive a card that looks like this with an issue date and expiration date. The cards are issued for 2 year periods. While in nursing school you will need to renew your CPR certification YEARLY.

11 Fit Testing Fit testing (respiratory mask fitting) is a required when you get to the clinical setting. Check SON website for clinical sites that require fit testing. Information is located under News & Information. Note: Typically completed during the first week of school –starting with semester 2. Not all clinical sites require fit testing. Valid for 1 year.

12 Flu Shots Due by October 15
(usually available between late August and Oct). Declination forms are available on the SON website located under Forms. Flu shots are valid from the date of shot until June 1 the following year. Hospital policies regarding declination of flu shot requires students to wear masks while in the site.

13 Quantiferon Tuberculosis Test
Due Yearly Obtain during: Dec 10-Jan 10 Or June or July TB can not expire during the semester or while in the clinical setting. Copy of lab results required to upload to CastleBranch. If you have a History of a Positive TB, you do not take the Quantiferon Blood Test- instead the following requirements are needed: 1. Chest x-ray is required within 12 months of starting the nursing program. 2. TB Assessment form ( located on SON website – Forms) This needs to be filled out by your primary care provider. RENEWAL: annually you will submit a TB Assessment form only. We do not accept TB skin tests.

14 Vaccinations Positive titer (preferred) in all three diseases or
Measles, Mumps, & Rubella Positive titer (preferred) in all three diseases or 2 previous vaccinations Hepatitis B Reactive titer or 3 previous vaccinations Non Reactive titers Require booster vaccinations and Varicella (chickenpox) Positive titer or 2 previous vaccinations Re-titer is required

15 Are you immune? (Titers)
Measles, Mumps and Rubella (MMR) Varicella (Chicken pox) Hepatitis B Requires blood draw Copy of lab results uploaded to CastleBranch Refer to Grid for course of action if titers are not Reactive – or Positive This is a blood test to see if you still have immunity from earlier vaccinations Titers are not vaccinations

16 Hep B MMR Varicella Polio Tdap TB Screening
Adapted from CDC Guidelines, 2014 Vaccination Requirements The Valley Foundation School of Nursing Vaccination Requirements Titer Follow up for No or Borderline immunity: Hep B Proof of (3) vaccinations or (+) Titer Reactive or Non- reactive Non Reactive titer – Proof of (3) previous vaccinations is required and revaccinate entire series of (3) vaccinations ( or PCP recommendations) and re-titer after 4 weeks ( or PCP recommendations) Re-titer Reactive– no further action required Re-titer Non Reactive– No further action required MMR Proof of (2) vaccinations after 2 years of age at least 28 days apart or (+) Positive titer- No further action required Negative titer – Proof of (2) MMR vaccinations in childhood are required and (1 or 2) MMR vaccination booster(s) should be obtained ( or PCP recommendation) No re-titer is required Varicella Proof of (2) vaccinations or (+) titer (+ ) Positive titer- No further action required Proof of (2) previous vaccinations and (1 or 2) Varicella booster(s) (or PCP recommendations), and Polio Proof of (3) vaccinations in childhood or Booster vaccination in adulthood (IPV) (+) is required only if no documentation is available Negative titer - IPV vaccination required –PCP to determine course of immunization No re-titer required Tdap Proof of current vaccination Tdap must be within 10 years after age 19 None  Td Every 10 years booster unless outbreak occurs. TB Screening Quantiferon Blood Test RESULTS Negative results – no further action required Positive results– Obtain chest x-ray and TB Assessment signed by PCP for clearance. Annual TB Assessment required yearly ( not testing required)

Copy of your yellow immunization or immigration card *Requirements are subject to change when a community outbreak occurs. Tdap (after 19 years old) Td booster every 10 years after*

18 NSO Liability ( Malpractice) Insurance
Please go to the SON website ( under policies) for information on obtaining liability insurance. Insurance is purchased yearly and should not expire during the academic year. Copy of Certificate of Insurance is uploaded to CastleBranch.
