2. Suffolk Community Healthcare (SCH)
The Community contract was awarded to a consortium of providers on
The consortium consists of WSFT, IHT, NCHC, Medequip and Bartrams.
This is part of wider health and care redesign to encourage integration and a partnership approach to patient care.
The contract was initially awarded for 12 months and has now been extended until September 2017.
Working in a complex and changing environment,
the focus remains on patient care

3. About the SCH Contract
SCH provides a range of NHS clinical services to the people of Suffolk:
15 community health teams – nursing and therapy care
Admission Prevention /Discharge Planning Services
A range of specialist nursing services eg COPD,
neurology nursing
Adult speech and language therapy
Four community hospitals
Minor injuries unit
Podiatry
Foot and ankle surgery
Care Co-ordination Centre (referrals management service)
Wheelchair services (Bartrams)
Community equipment services (Medequip)
Back office services include:
