1 Welcome to the Suffolk Community Healthcare (SCH) Staff Induction A service delivered by a partnership of: West Suffolk NHS Foundation Trust (WSFT) The Ipswich Hospital NHS Trust (IHT) and Norfolk Community Health and Care NHS Trust (NCHC) .

2 Suffolk Community Healthcare (SCH)
The Community contract was awarded to a consortium of providers on The consortium consists of WSFT, IHT, NCHC, Medequip and Bartrams. This is part of wider health and care redesign to encourage integration and a partnership approach to patient care. The contract was initially awarded for 12 months and has now been extended until September 2017. Working in a complex and changing environment, the focus remains on patient care

3 About the SCH Contract SCH provides a range of NHS clinical services to the people of Suffolk: 15 community health teams – nursing and therapy care Admission Prevention /Discharge Planning Services A range of specialist nursing services eg COPD, neurology nursing Adult speech and language therapy Four community hospitals Minor injuries unit Podiatry Foot and ankle surgery Care Co-ordination Centre (referrals management service) Wheelchair services (Bartrams) Community equipment services (Medequip) Back office services include: Quality and Governance Workforce Performance/IM&T Facilities Business Support and Administration Paediatric Services: Medical Team Physiotherapy Team Occupational Therapy Team Speech & Language Therapy Team Community Nursing Team Community Audiology & Newborn Hearing Screening Clinical Psychological Therapy Team

4 Facts and Figures Employees of SCH
Total number of staff employed within the SCH contract is 1221. Breakdown as follows IHT – 238 NCHC – 556 WSFT – 427 From January to December 2015 we had: 608,955 Face to face contacts with patients (Inc. Phlebotomy) 124,091 New referrals for all services 1068 Admissions to community hospitals 74,173 Telephone contacts for all services
