1 Mums and Kids Matter Program
Wesley Mission is managing this new statewide community based program, funded until 30 June 2016 through the National Partnership Agreement on Supporting National Mental Health Reform. The residential is based in Sadlier, South Western Sydney LHD

2 What does the Program offer?
Will assist mothers with mental illness and their children (0-5 years) by providing mental health and parenting support, whilst also catering for needs related to physical health, accommodation, vocation/education, disability and other co-morbidities. contribute essential components to the capacity for providing mental health care for mothers and their young children in NSW

3 Key Service Components
Residential (8 units): short-term (up to 3 months) non-acute residential mental health and tertiary parenting care services (staffing and setting 24 hour/7days per week care) Accommodation Support Packages: in-home or residential care accommodation and support packages for mothers and their children. Brokerage for Specialist Care Packages: specialist care packages for either hospital inpatient mother-infant admissions (eg St John of God Burwood) or supported residential admissions (eg Karitane or Tresillian).

4 Who is eligible for the Program?
current client of a NSW public mental health service has a current care plan at the time of referral mother with mental illness at least 16 years of age living with a complex mental health problem (including physical health issues and other co-morbidities) that prevents them living in the community with their infant child/young children capacity to provide informed consent to participate in the Program residential stays are only available for mothers and their children who are less than 5 years of age specialist care packages (in home/community) are available for mothers and their young children (0-5yrs) who may also have older children living with them Inpatient admission in line with facility acceptance criteria.

5 Who is prioritised? Mothers who:
Are in hospital due to their high support needs; Have been separated from their young children due to complex mental health problems; Are homeless, at risk of homelessness or live in inappropriate housing; and/or Find it difficult to maintain stable tenancy without high level support.
