Number and type of Patients/Residents/Clients: 2 Geriatric Mental Health Inpatient Units. Each unit has 20 beds. Clients are 60 years and older living with a mental illness and or addiction issues.

4 AIM To reduce injury from falls by 40% from baseline by March, 2011.
Increase the amount of clients identified “at risk” for falls to have a documented falls prevention intervention strategy (100%) by March 2011. Increase the amount of clients screened using a “falls risk assessment form” upon admission (100%) by March, 2011.

Lessons Learned/Key Insights Communication, communication, communication Dissemination of information can be difficult for a large organization The evaluation tools were helpful in providing insights into the effectiveness of our Falls Prevention Program. (in effect since, 2007). (process and outcome measures) PDSA cycles assisted us in planning change and making the work feel manageable.

Key Sustainability Steps/Plan: Target Dates Continue to track rate of falls causing injury (decrease 10%) June, 2011 Continue to track the amount of clients assessed at admission for risk of falls. (100%) Continue to track the amount of clients at risk for falls with a documented falls prevention care plan. (100%)
