Increase the amount of clients identified “at risk” for falls to have a documented falls prevention intervention strategy (100%) by March 2011.
Increase the amount of clients screened using a “falls risk assessment form” upon admission (100%) by March, 2011.

The evaluation tools were helpful in providing insights into the effectiveness of our Falls Prevention Program. (in effect since, 2007). (process and outcome measures)
PDSA cycles assisted us in planning change and making the work feel manageable.
