3 3 Three Parts to our Project: 1.Creating networks of contact people in local agencies and institutions; 2.Identifying the roles and learning needs of health care providers to effectively support community organizations; 3.Identifying the roles and learning needs of health care providers to effectively support patient self management of their health and health care needs.

4 4 Part One – Create Networks Purpose: To create a network of local agency contacts comprised of representatives from schools, municipalities, and health authorities as a platform for knowledge and information sharing So What? –As a platform for information and knowledge sharing Now What? –A list of key contacts covering local municipalities, school regions and health authorities (public health system) is currently being developed.

5 5 Part Two – Establish Network Nodes1 & Identify Organization Roles and Learning needs Purpose: Once the network of some local nodes between schools, municipalities, and health authorities has been established as platform for knowledge and information sharing, these nodes will be asked to identify the roles and learning needs of the organizations and their representatives with respect to the ‘health literacy’ agenda. What? –Identifying health care provider learning needs So What? –To enhance their ability to support community agencies. Now What? –Identifying competencies and roles of health care providers.

6 6 Part Three – Supporting Patient Self-Management Strategies Purpose: To identify the roles and learning needs of health care providers in support of patient self-management strategies, including health literacy So What? –To enhance their ability to support patients Now What? –Identify competencies and roles of health care providers
