3 New CHO Structure Structural reform as an enabler 9 CHOs 90 Community Healthcare Networks 450 Primary Care Teams (5 on avg. per CHN) Team leaders and key workers strong relationsips with local communities responsible for service delivery and integration with specialist services greater participation by GPs standardized clinical governance Delivering improved integrated care through a multi disciplinary management team Bringing more autonomy and decision- making ability to local level Consultatvie fora at National, CHO & Network levels Community Connectors Linking people to local formal and informal netwroks & services CHOS Networks Each Network Serving: 50,000 (avg) 30 -70,000 Range Community Development approach to support voluntary sector participation

4 Community Healthcare Organisations Reform Programme: New Structures: Oct 2014 - Community Healthcare Organisations – Report & Recommendations of Integrated Service Area Review Group launched Dec 2014 - National CHO Steering Group Established Jan 2015 - Chief Officers appointed Mar 2015 - CHO Implementation planning exercise commenced Dec 2015 - CHO Implementation Plan finalised Mar 2016 – CHO networks mapping completed June 2016 – CHO Management Teams take up duty including Head of Social Care

6 Transforming Lives – The Challenge of Change 6 ‘New Directions ‘...for our Day Services 18,000 people with Disabilities use our Day Services 900 Day Service Locations €300m 73 Providers ‘Time to Move On’.. to better Residential Services &options 8,800 people with Disabilities supported in Residential settings 2,500 people living in a congregated settings 6,300 people living in Community settings Challenges  People with disabilities to have high expectations for themselves and their community.  Families and services to adopt an attitude of positive risk taking  Blurring of boundaries between special and mainstream services  Service providers to deliver better & innovative outcomes from new & current service models.  HSE to review how services are funded and to introduce new and innovative ways of funding services that support choice, person centredness, accountability and best value.
