1 June 2016 SEN Services SENCO Training Preparing for Adulthood

2 SEN Code of Practice Chapter 8: Preparing for adulthood from the earliest years This chapter sets out how professionals across education, health and social care should support children and young people with SEND to prepare them for adult life, and help them go on to achieve the best outcomes in employment, independent living, health and community participation.

3 Key Points: Local Authorities must ensure that the EHC Plan review at Year 9, and every review thereafter, includes a focus on preparing for adulthood. All schools have a statutory duty to ensure pupils from year 8 to 13 are provided with independent careers advice That the Local Authority must name the College in the EHCP by 15 March prior to the September start date

4 Development of Preparing for Adulthood and Transition Transitions event at Victoria Hall, Saltaire on the 20 May 2016 attended by 80 schools, colleges, health service and local authority representatives. Agreement to hold a sub group of representatives of schools and colleges to develop the preparing for adulthood paperwork and transitions process, held on 10 June 2016.

5 Management of PFA Reviews – Year 9 The annual review in year 9 will include an element of preparing for adulthood; schools are requested to focus on: What is the young person to achieve for the rest of their time in school? What is the young person hoping to do when they leave school? What help or advice is the young person likely to need to make an informed decision about choices after school? Any other relevant information e.g. travel training, independent living skills, functional skills.

6 Management of PFA Reviews – Year 10 The annual review in year 10 will include an element of preparing for adulthood the focus is: Learning and Employment Related skills Interpersonal skills Independence, Life skills and Community Living skills Health and Psychological Well Being In addition: – the young person/parent will make a preference for Post 16 provision –future actions to be taken including the involvement of other agencies and providers e.g. College in the year 11 review.

7 Management of PFA Reviews – Year 11 The annual review in year 11 should be held in the Autumn Term and the Preparing for Adulthood Conversion Annual Review paperwork should be completed. The College or identified destination should be invited and attend the review to ensure the EHCP (PFA) includes the needs, outcomes and provision for Post 16. In cases where the intended destination is the current or another school the Preparing for Adulthood paperwork must be completed. SEN preference form completed.

8 Actions to support Preparing for Adulthood Transition Local Offer site to be reviewed for College provision and offer Colleges to receive a list of all young people in Yr9 onwards with range and primary need to support planning of provision Directory of School and College staff to be pulled together and circulated/BSO.

9 Actions to support Preparing for Adulthood Transition Integrate PFA into the annual review documentation Add preference to the annual review paperwork All Year 11 annual reviews are held in the Autumn Term Consultation with Colleges Jan/Feb – options to be specified in the EHCP.

10 Next Steps: completed by 24/6/16 1.Update all annual review and preparing for adulthood paperwork 2.Alignment of areas of need to PFA to be available on BSO (Nazam Hussain, EP) 3.Bradford Schools on line to be updated and a new PFA Review area to be in place 4.Directory of school and college staff to be published 5.Inform colleges of yearr 9 onwards pupil data. 6.Prepare transition information for 2017
