3 The POPP Organisation The Funding: DH funding £2.4m yrs 1 & 2 (2006 – 2008) DCC/NHS Dorset funding - £1.28m yrs 3 & 4 (2008 – 2010) - £800,000 yr 5 (2010 – 2011) The Vision: Build supportive neighbourhoods for older people Seek transitional change to existing ways of working Focus on prevention, health, well-being, and independence The Demographics: Older People aged 50+ living in Dorset excluding Bournemouth and Poole (179,000+)

4 Strategic Direction and Decision Making Programme Management Community Development and Capacity Building Wayfinders (x54) Signpost and Support – 9 hours per week Talks, events, promotions, practical tasks Leaders (x33) Challenge and change – 7 hours per week Work with older people and others to identify and develop local opportunities based on need Evaluators (x10) Capturing the journey Community Commissioning Fund Seed funding to kick start local initiatives and activities

5 Community Development Workers Essential to the programme - Supporting Leaders and Wayfinders Helping people to work together Input into meetings, events, and Locality Planning Work with organisations to influence change Assist in the completion of applications for POPP funding Develop activities and services with Older People Partnership working and developing networks A link between communities and DCC/NHS Dorset Support the development of DCC and partner agency strategies Thematic leads: BME, Faith, Housing, Art, Culture and Heritage, Mental Health, and Caring issues Audits - Home from Hospital Services / Exercise and Activity Opportunities within Sheltered Housing

11 The Dorset POPP Vision and Desired Outcomes “Supportive communities that enable older people to remain living in their own homes for as long as they wish”. Desired Outcomes of the Dorset POPP: Older People in Communities… …have housing suitable for individual needs …are socially integrated and not isolated …are making a positive contribution and experiencing fulfilment …feel secure and safe …feel free from discrimination …feel financially secure …are in good health in mind and body …have dignity, choice and control throughout their life

12 New Initiatives and development Access to Food and Nutrition (TSC Funded) Work with the individual needs of existing Meals on Wheels recipients to identify appropriate opportunities for access to food and meals according to their need Map existing lunch clubs and other communal eating opportunities Develop new opportunities for access to food and meals by supporting emerging lunch clubs, opportunities through Day Centres, Schools, Pubs and Clubs etc. Work with older people and local agencies to find solutions to identified needs. Ensure the needs of excluded individuals and groups are identified and met as part of this process.

13 New Initiatives and development Volunteer Driver and Car Scheme (LAA Funded) Map existing community transport and volunteer driver/car schemes Co-ordinate, develop and support new opportunities for volunteer driver and car schemes to emerge in Dorset settlements. Work with older people and local agencies to find solutions to identified needs. Ensure the needs of excluded individuals and groups are identified and met as part of this process.
