Most Common Issues Course Completions Discipline Foundation High School Program – assigned to all students beginning the 9 th Grade in 2014-2015 and those who opted into the program Career and Technology Contact Hours Dual Credit College Hours

Course Completions Entire Campus missing Course Completion Records – Edit 41556 Elementary Grade Levels that use a Skills Based Report Card missing Pass/Fail Indicators – all courses had a code of 00 – course not completed Report used to see these issues was PRF7D006 – Number of Students Completing Courses by Pass/Fail Indicator – Elementary courses have Pass/Fail Indicator of 13 (passed) or 14 (failed) and Secondary courses have Pass/Fail Indicator of 01 (passed) or 02 (failed)

Discipline The mandated action was not taken by the district or was taken but the correct reason code was not reported Incident occurred at the end of the year and the actual length served was not changed to reflect the actual number of days with the appropriate difference code assigned Partial day ISS or OSS assigned for more than one day

Foundation High School Program All students entering the 9 th Grade in the 2014-2015 school year must be coding as graduation with the Foundation High School Program and the endorsement they are pursuing Any students who have opted into the Foundation High School Program must also be reported Report PRF7D082 – Student Foundation High School Program Roster by Grade

Career and Tech Contact Hours On report PRF7D007 – Comparison of Current and Prior Year Attendance is there a significant difference reported on Career and Tech FTE If there was a significant difference we asked the districts if there were changes in the number of CT courses offered, number of students taking CT courses and to please double check the contact hours assigned to the courses

Dual Credit College Hours Looked at report PRF7D079 – Students with Dual Credit Courses/College Credit Hours Make sure that there are college hours reported for courses that were passed
