8 Action planning… Key principles: We will be outcome focussed We will build on what we already do We will try new things We will try to be consistent across the organisation It’s about relationships as well as structures We will evaluate what we do and learn from our experience It’s everyone’s responsibility Everything we write will be “easy read”

9 Outcomes Public involvement is a part of what we do every day and we consistently involve people in our work People can easily find out what the health and social care partnership is, what it does, and how to get involved People feel they are able to be involved if they want to and that they can make a difference We have a strong relationship with the community groups and networks that support people to have a voice More people choose to be involved in our work including people who find it harder to participate or to be heard We provide better care for people because we respond to ongoing feedback from a wide range of people We make decisions with people and we can show how working with people has made a difference

12 Looking at what information tell us and thinking about what we need to do Writing a draft Strategic Plan Consulting on the plan 1. We will provide advice about the key themes, tone, language of the plan, how to make it relevant and real 2. We will make sure the outcomes of the consultation are reflected in the plan 3. We will help to create a communications and engagement plan The role of the Community Collaborative  There will be three meetings of the Community Collaborative  We will tell people what difference the Community Collaborative has made to the Strategic Plan. Consulting and gathering information Producing a Strategic Commissioning Plan
