2 Question Cards There are stacks of Question Cards on each table. If you have any questions for the speakers, please write your questions on the cards. Cards will be collected at the conclusion of each speaker’s presentation. If you need more question cards, please contact the staff member at the registration desk.

3 Course Evaluations A course evaluation was given to you when you checked in at the registration desk. If you haven’t already, please take a moment to think about what you expect from this CME activity and complete the “Before” portion of the front page before the presentations begin. Please make sure your pre-printed information (name, address, etc.) is correct and note any changes. Course evaluations are to be completed and returned at the end of the meeting.

4 CME Certificates Course Certificates will be distributed at the end of the meeting. You must turn in your course evaluation in order to receive your certificate.

5 Evidence-Based CME This event has been approved for 3.5 credits of Evidence-Based CME credit by the AAFP. This means that family physicians and physician assistants will receive up to 7.0 credits for participating in this event.

6 Continuum of Learning This activity is part of the Antidote Continuum of Learning™ platform. This live activity will have related online components in the form of an online case study (hypertension) and presentation (HPV Vaccine). You will receive more information about the online components at a later date. Antidote believes education is not a one time event and we are applying that to all of our offerings, providing you additional opportunities to earn credits while helping you improve your practice and patient care.

7 Audience Response System You will find a small keypad at each place setting. These are for the Audience Response System (ARS). You will be asked questions throughout the presentations and will utilize the keypads to answer the questions. Answers will be tabulated and displayed on the screen. Please do not remove the keypads from the tables.
