5 Enter grade you expect to achieve or have already achieved and indicate if exams taken were at higher or lower level Enter subjects taken in final examinations Page 4 of 13 2 nd Level or Final High School Results Enter Examination Type, e.g A’level, IB, Leaving Certificate etc

6 Page 5 of 13 3rd Level or University level qualifications. For Applicants applying on the basis of a Primary Degree.

8 Page 7 of 13 Insert contact details of person who will act as an academic referee. This is usually a school counsellor, teacher or professor. If you wish to add an additional referee you have to option to enter their contact details Page 7 of 13 At least one of your Academic referees contact details is mandatory. Either your Schools Guidance Counsellor or Principals details should be entered in 1 st Referees details. If you wish to include another referees details it can be entered in 2 nd and 3 rd Referee Contact details of either your schools Guidance Counsellor or School Principal to be entered in 1 st Referee

9 Page 8 of 13 This section allows you to upload the supporting documents required to complete your application file. If it is more convenient the supporting documents can be sent to the Admissions Office by e-mail or post quoting your application number. If the files are on your PC and you wish to upload them; locate the appropriate file using the ‘Browse’ button. Then click ‘Upload’. Do this for each document you wish to upload. The upload time will vary depending on the size of your document and the speed of your internet connection. It is recommended that you keep these files as small as possible A green check will indicate when a document has been successfully uploaded

10 Page 9 of 13 Insert additional information as required. Free text boxes have a maximum size and will stop accepting characters when full. If you wish to add additional information you may send it to admissions@rcsi.ie quoting your application number.admissions@rcsi.ie Your application number is on the printed version of this form available on the last page of the online application.

12 Page 11 of 13 Summary of all your details entered on your application form. Once you are happy that all details entered are correct please tick the box to declare the form was completed accurately.

It is important you print this page as the printed version will contain your application number which you need when corresponding with the Admissions Office or when submitting additional documents to your application PRINT BUTTON
