To learn how referrals from the GP could be easier To support GPs to have better information to support carers To support 50 beneficiaries

Lincolnshire Carers and Young Carers Partnership Ltd Registered in England No. 08548267 Registered Office: Companies House The Process Carers developed a wellbeing questionnaire for Carers. 125 responses received 30 carers involved in the formation of this project LCYCP& carers developed a surgery questionnaire Wellbeing working group established, meeting 10 times across the county Carers developed leaflet to help remove the stigma associated with mental health

Over 45% stated that having more recognition and support from professionals would help Meeting up with friends would help reduce the impact Meeting up with people in a similar position Having more ‘me’ time Having more time to do household tasks Having transport available would reduce isolation

Only a quarter of these had an up to date carers register Some used this to target carers with information 4 Patient Practitioner Groups actively involved Two Clinical Commissioning Groups now actively involved

Carers need to tell the GP they are a carer Surgeries raising concerns about where to find information to support carers Some surgeries requested external support to improve services offered Surgeries are telling us they want better access to information about carers issues. Lincolnshire Carers and Young Carers Partnership Ltd Registered in England No. 08548267 Registered Office: Companies House Conclusions Carers feeling isolated only 32% of carers surveyed met people less than once a month A further 10% never met anyone. improved social integration has positive benefits on peoples mental well being There potentially are some areas to develop stronger safeguarding practises for Carers. There were a significant number who have disturbed sleep due to their caring role, or who were verbally or physically abused

Lincolnshire Carers and Young Carers Partnership Ltd Registered in England No. 08548267 Registered Office: Companies House Conclusions The Caring role can lead, in a large proportion of cases, to negative thoughts leading to potential mental illnesses – could be supported through low level counselling. Peer Support -Carers strongly acknowledge the role of peer support in supporting the caring role. Attendance at peer support groups result in larger social support networks, enhanced self esteem and social functioning, thus improving mental wellbeing and enabling carers to self direct. Carers not being informed as they do not necessarily know where to find information results in an exacerbation of poor mental wellbeing

GPs need information on the support required for patients. Better information needs to be available to Carers at Surgeries.

Analyse the information from both carers and healthcare professionals in more detail Promote the Carer Journal Promote an information pack that supports GPs to help the carer more readily Report findings to Lincolnshire Partnership Foundation Trust and Clinical Commissioning Groups Influence strategic decisions with this evidence and this information will support the work of the Joint Strategic Needs Assessment Launch new project to support carers emotional resilience

Research demonstrates that volunteering leads to better health and that older volunteers are the most likely to receive physical and mental health benefits from their volunteer activities.

The support that people with lived experience can give one another. In a support group, people are able to talk with others who are like themselves - people who truly understand what they're going through and can share the type of practical insights that can only come from first hand experience Research has shown that peer-run groups result in larger social support networks and enhanced self-esteem and social functioning. We will do this through Peer Listening, Peer Education and Peer Role Modelling.

Lead by the members of the groups, what the group want to do. Structured around practical sessions supporting healthy minds and bodies.
