16.  Provincial PPTICRMs in some provinces not formally appointed resulting in low levels of commitment.  Ideal Clinic activities are not seen as part of core responsibilities.  District champions are also responsible for other programs, e.g. Risk Management, MCWH,NCS, Infection control and complaints managements,etc.  Team members are allocated other ad hoc responsibilities by their immediate supervisors Challenges of PPTICRMs

17. CONCLUSION  The is a need for provinces/districts to operationalise resolutions of the Operation Phakisa ICRM Lab as approved by Departments of Health and The Presidency.
